  2. The Session Objectives • to explain what is specific for inspection of SSCs important to safety during maintenance • explanation of the role of site inspector during outage • to present some examples of inspection findings during maintenance

  4. Specific of inspection of SSCs during maintenance • many activities on SSCs during outage: • planed maintenance • unplanned maintenance • modification • testing • plant configuration is specific for outage / shut down mode • time stress, short time for conduction and evaluation of inspection

Role of site inspector • every day report to headquarter • to be fully aware with ongoing outage works • to inspect vendors on the site /work permit, internal contact person…/ • to inspect implementation of the modifications • photo documentation if necessary • member of inspection team (if possible) • to initiate special inspection for area in which site inspector is not specialized

  6. Conduct of inspection (1) • Inspection is performed based on certain inspection manual. • If inspection is not covered by manual special program is prepared. • During inspection of test of SSCs important safety pay attention to conformity of the actual plant configuration with initial status of the SSCs considered in the test procedure. • Proper implementation of modification of SSCs include also modification of relevant documentation/ procedures and adequate staff training. • Focus on verification of fulfillment of acceptance criteria during test of SSC especially during the test after modification.

  7. Conduct of inspection (2) • Make written notes for yourself to be able to write inspection protocol properly. • Ask for site inspector support if necessary.

  8. Start up after outage • Before bringing an installation back into operation following a refueling outage, major maintenance operation or a long term shutdown, the operator should demonstrate to the regulatory body that the installation will be able to continue to operate in compliance with the safety requirements. • The regulatory body may approve or agree to go back into operation and attach conditions as appropriate. • Documents demonstrate readiness of SSCs and whole NPP as well for start up is reviewed by regulatory body.

Examples of inspection findings during maintenance (1) • Identification of findings: outage, site inspector walk down • Problem: feed water pump was repaired, working place was not labeled properly in compliance with internal procedure • Inspector activity: notify, make note (time, place, number of work permit...) • Result: problem was fixed by operator

Examples of inspection findings during maintenance (2) • Identification of findings: outage, event report • Problem: working glove on the sealing part on the opened reactor • UJD activity: inspection of licensee activities before event (all working tools are registered except working gloves) • Result: procedure/system for tools registrations was modified

Examples of inspection findings during maintenance (3) • Identification of findings: outage, event report • Problem: 13 fuel assemblies loaded in the reactor core rotated by 180° (discovered during final core loading check) • UJD activity: inspection aimed at investigation of causes of the event (human factor) • Result: involved personnel retrained from the proper procedures

Examples of inspection findings during maintenance (4) • Identification of findings: outage, start up, planed inspection • Problem: diesel generator failed to start during the test, hardware problem (rotation speed meter) • UJD activity: inspection of maintenance activities during outage • Result: modification of procedure

Examples of inspection findings during maintenance (5) • Identification of findings: outage, start up, planed inspection • Problem: Several modified operational procedures not present at the working place (MCR) during plant start-up. Several were present but the personnel was not acquainted with them in a provable way. • UJD activity: Inspection findings with immediate corrective actions formulated. • Result: Start-up halted until all new procedures were put in place and personnel was acquainted with them.
