Physician Outreach in Peel • Regular communication and outreach to Peel physicians occurs on a number of topics, including Immunization, Obesity prevention, Environmental issues, and Public Health Program information and referral pathways • Physician Outreach is strategic and evidence-informed

  3. What does the Literature say about effective physician outreach? The evidence shows strongly that; ‘print materials alone have small effect’ for professional outcomes and no effect for patient outcomes.

  6. 18 Month Well Baby Promotion • 18 Month Well Baby Physician Package: • Letter from Medical Officer of Health promoting the screening • Information for Physicians and Health Professionals • Rourke and 18 Month Nipissing • Peel Pathway

  7. 18 Month Well Baby Promotion Physician Rounds: • Brampton Civic, Trillium and Credit Valley hospital Family Practice Rounds • Presentation was offered to Chiefs of Paeds • Family Practice Physicians were provided a USB key with resources and links to Offord Centre site • Physician resource website is under construction

  8. The Healthy Babies Healthy Children Program and community partners on the Early Identification Committee worked together to develop the Peel Pathway. Pathway will be revised, as needed, by the Early Identification Committee. The Early Identification Committee is Co Chaired by Peel Public Health and Infant Child Development Services of Peel (ICDSP)The Early Identification Committee meets quarterly and promotes agency collaboration and resource development in addition to the 18 Month Well Baby Pathway

HBHC Program - Early Childhood ScreeningSix Weeks to Six Years • Goal to identify and link children to services before school entry • Lead with Early Identification Advisory Committee for Peel • Distribution and promotion of Nipissing Parent Screening tool • Working with physicians & community partners • - PEP-Start Clinics - Improving the Odds workshops
