  3. Mission: To “walk with” seniors in transition and their family caregivers to support health in spirit, mind, and body

  4. 5 part-time Staff & 100+ Volunteers make the Center work!

  5. Core Service: Volunteer help Driver volunteers and volunteer care teams for errands

  6. Core Service: Parish Nurse organizes Health education, wellness, screening & prevention

  7. Core Service: Resource Coordination Information & referral includes one-on-one meetings with senior and his/her family to discuss options

  8. Our 12 member Board of Directors guide and inspire . . .

  9. From a Pilot with volunteers giving rides to 18 elders in 1998, the Center now serves over 540 people a year.

  10. Grant funded collaborative project that started six years ago.

  25. Partners in Care Resource Tool Feature: • Personalized “Partners in Care” • List of family members, caregivers, helpers, and emergency contacts • Agencies you choose to list can review personal profile to understand issues and respond more proactively to requests

My Partners in Care • Like social networkingconcept, but on a small scale and focused on maintaining independence and/or finding help • The family, friends, neighbors, yard-work person, home care provider, or others are “Partners” • Contact information – handy for family • Add to the list as desired

  27. Partners in Care Resource Tool Features: Tailored Assessment, Action Plan and Profile • Each senior completes the personal Self-assessment • Responses generate an Action plan with suggested steps • Tool automatically creates a Profile of the person based on the assessment

  28. My Assessment • Provides a snapshot of current status • Complete one once every 6-9 months or when there is significant change in condition • Supplements your own ideas and observations

  30. Action Plan • Is automatically generated based on person’s responses to the assessment questions • Is based on Normandale’s actual experience in “walking with” elders and family members • Symbol of traffic light used to indicate areas of focus/concern and strengths

  31. Partners in Care Resource Tool Features: • Communication • Senior, Family members, and Helpers can add a note to the Notes section • If listed in the “Partners in Care,” a staff member from an agency or other organization can also view the notes and see the person’s profile.

  32. Partners in Care Resource Tool Features: • Education and Resource Directory • Direct connection to state’s database of resources • Educational materials to improve health, nutrition, exercise and independence

  33. Resources Directly links to the state of Minnesota live database of over 37,000 resources (separate small window pops up on screen)

  36. Project Timeframe • Project continues through end of 2010 • We hope every senior learns new things related to wellness and prevention • Looking for feedback on how the electronic resource tool has helped seniors and families (how it was used, suggestions for changes)
