Process (exploring current services) • Prepare one-swim lane for each service represented • Review case study with participants, adding or changing features to ensure all participants can engage • Agree key events • Each participant prepares post-its summarising how they identify, assess, treat/intervene, refer on • Review service by service and overall, to ensure all participants understand all service’s involvement with the young person • Add green flags for effective service • Add red flags for poor service, identifying gaps and overlaps, where integrated working could be improved to provide better outcomes

  5. Examples of Intervention to support GEM and her family • Utilise youth workers to make initial contact with Gem – Lead Professional • School – call a case conference with agencies to get a clear picture of Gem’s present situation (CAF) • Utilise Education Welfare to make contact at home – link to police through Truancy Rapid Response Team • Utilise Youth Service to work with group in danger of the ASBO – get them involved in more positive activity • Connexions – GEM need ARIR assessment + support to ensure she doesn’t become NEET • Utilise school nurse to talk through GEM’s respiratory problems • Social Services – is there a potential child protection issue – does the family need some concentrated support • Work with YOS to prevent an ASBO being implemented
