Population cible • Patients atteints de : • Polyarthrite rhumatoïde • Spondylarthrite ankylosante • Rhumatisme psoriasique • ostéoporose • Sous biothérapie IV ou sous cutanée : • etanercept (ENBREL®), adalimumab (HUMIRA®) • Infliximab (REMICADE®) • Rituximab (MABTHERA®) • CTLA4 Ig (ORENCIA®) • Tocilizumab (ROACTEMRA ®) • Patients ostéoporotiques traités par parathormone (FORSTEO®) : sous cutanée. IV

  3. Moyens humains • Moyens infirmiers : • Éducation assurée par une seule IDE dans le service. Poste de consultation (30% consultation, 70% éducation). • Formation d’une IDE pour assurer la continuité • Sensibilisation et formation des IDE d’HDJ • Moyens médicaux : • 2 médecins du service suivent actuellement des formations • Multiples intervenants formant « un réseau » : • Kiné, ergothérapeute, diéteticienne, assistante sociale…

Éducation : méthode Diagnostic educatif + bilan prébiothérapie Consultation 45 min individuelle HDJ CS Biothérapie sous cutanée : 1ère injection + apprentissage à l’auto-injection + compétence d’auto-sécurité Biothérapie IV 1ère perfusion + Compétence d’auto-sécurité 45 min individuelle individuelle individuelle Séances d’éducation thématiques 45 min 1h30 collective AMBULATOIRE
