  1. System Safety Society Eastern Canada Chapter - presented by Bob Fletcher - on 22 August 2005

  2. Eastern Canada Chapter • Goal • The goal of the Eastern Canada Chapter is to develop within our geographical area a professional focus on high quality system safety information to increase the competence and effectiveness of the people working in the System Safety profession.

  3. Activities 2004 / 2005 • held seven, lunch time events and one, half day event • developed a brochure to assist with advertising and promoting the Chapter • developed and distributed a survey form • developed a local web site, • contacted several related organisations that host similar events and discussed joint events • built rapport within local community to assist with future events • provided support to the 2006 ISS conference • video taped presentations and made DVDs to distibute

Activities 2004 / 2005 • Participation in the Society’s awards program - no nominations this year; possibly next year • Administrative meetings - 10 • New members to the Chapter - fell from 40 to 25 since free 1 year memberships were given at the conference; expect new members to sign up next year • Elections - spring 2006 • Articles - none, plan to start a newsletter in October 2005 • Financial status of the Eastern Canada Chapter - next slide

Financial Summary - FY2004 / 2005 CDN $ USD $ • Start - Account Balance, July 1 of FY $ 312.09Note 1$ 256.20 • Income for FY (Not including member reimbursement) $11,850.83 $ 9,728.46 • Income from member reimbursement $ 286.63 $ 235.30 • Expenses for FY $ 3,674.64 $ 3,016.55 • End - Account Balance, June 30 of FY $ 8,774.91Note 2$7,203.41 Note 3 Note 1: Actual account balance is $1227.94 (but all are ISSC21 funds) so $0.00 are actual chapter funds. $312.09 is shown here (the amount of Petty Cash held in Chapter Cash Box) Note 2: Actual account balance is $84.00 less (the amount of Petty Cash held in Chapter Cash Box - the two accounts were totaled for Net Worth) Note 3: US value is given for information only for comparison with other chapter expenses. ECC funds are tracked and accounted for in $CDN. The two payments from HQ - $6818.74 CDN for ISSC21 ($5,880 USD) and $286.63 ($250.00 USD) for membership reimbursement are actuals based on bank deposits.

  7. 7 lunch time events (2) • “The Mid-air Collision over Constance Lake, Germany” by Kathy Fox, Vice President – Operations, NAV CANADA. Reason's model was used to identify some of the issues that contributed to this accident as well as lessons learned. • “Accident Investigation Without The Accident” with Mr. Michael Poole,P.Eng., Flightscape Inc. • "Safety Within Healthcare" with Alan Forster, M.D., FRCPC, MSc.

Conclusion • The Eastern Canada Chapter • had a good first year • the future looks good for continued growth • will require dedication, commitment, persistence and hard work to reach the full potential of the Chapter • team of volunteers have the skills and commitment needed to develop the Eastern Canada Chapter during the coming year
