2. What are S.O.Fs? • Forms that allow staff members to access quad community activity funds. • Forms that act as “money” at specific restaurants and stores • Forms that allow staff members to purchase necessary items without using their own money (which you can get refunded for, tax not included)

3. When Would RAs Use S.O.Fs? • Preparing for a floor or community program • Food • Materials • Supplies • Speakers • Etc • Preparing for a C.D.A • Stress relief goody bags • Craft supplies • Etc

4. When Would HRs Use S.O.Fs? • Preparing for a community wide program or when helping prepare for a floor program • Preparing for staff bonding activities or meals • Preparing for staff thank yous

5. S.O.Fs: The ‘Before’ Process • RAs discuss ideas with HR and get verbal approval • Complete a Request for Short Order Form (located in the service center) • Needs HR signature • Submit request to the quad AD • At least 3 days before the SOF is needed • Pick up SOF form from RA/HR box (an email will be sent) • Check which locations accept SOFs

6. S.O.Fs:The ‘During’ Process • SOFs should be used at participating vendors (only certain stores accept SOFs) • Before purchasing, alert the vendor cashier or waiter that the bill will be paid with a DU purchase order • Remind them that DU is exempt from any tax • On the SOF • The place of purchase, the date, general # of items purchased, & the total purchase price • Provide as much information as possible!

7. S.O.F.s:The After Process • Take the yellow, pink, & green copies of the SOF & the original store receipt and return to the AD (stapled together) • within the next 48 hours • NOTE: a SOF has 4 copies- • White: Vendor Keeps • Pink: Office Copy (school needs) • Yellow: Accounting Office (school needs) • Green: Purchasing Office (school needs)
