Your “Community” • For purposes of this HIT Toolkit for Small and Rural Communities, “community” is defined as organizations that are legally related to one another within what is often referred to as a healthcare delivery system or healthcare delivery network • Your “community” may include a hospital, physician practices that the hospital owns, a long term care facility, assisted living facilities, home health agency, DME provider, ambulance service, and other facilities under common ownership • Your acquisition of this HIT Toolkit is your license to use the tools within your “community”
