"Good" lets review your report of findings that will explain what we found on the examination that performed (yesterday) _________. 2. Reason for the Report • “We consider the "report of findings" to be one of the most important visits in our office and is important to understanding your problem and the decisions you will be asked to make regarding your health.” 3. Cause, Severity and Prognosis • At this time, review the cause, the severity, and the prospects for correction of the subluxation. • Use the "Report of Findings" form to guide you in the explanation of the patient's examination facts. Also consider any past history information and relate it into the report of findings.
