Members will be required to actively elect benefits online for: Medical Vision Life and Disability Health Care Flexible Spending Account Dependent Care Flexible Spending Account If members do not take action to dental benefits, current elections will automatically continue. 6

Benefit Changes – Vision Premium reduction with a 4-tier plan structure LASIK allowance increase from $300 to $600 (“in lieu of” limitation removed) Hearing Discount Plan available at no cost for all eligible employees and their dependents Access to EPIC Hearing Health Care national network Savings on all name brand hearing aid technology 10

Benefit Changes – Life and AD&D Definition for Wavier of Premium provision while disabled revised from ‘Own Occupation’ to ‘Any Occupation’ At the end of the elimination period, you may qualify for Waiver of Premium if you are prevented from performing one or more essential duties of Any Occupation ASRS participants will be required to contact The Hartford for Waiver of Premium claim filing Change will only affect LTD claims with dates of disability on or after January 1, 2015 12

Benefit Changes – Life and AD&D Supplemental Life Insurance/AD&D Premium reductions Increased coverage amount from $300,000 to $500,000 or up to 3 X annual salary For 2015 only, coverage can be increased in increments of $5,000 up to the plan maximum; future enrollments will be limited in an increase of $20,000 Dependent Life Insurance/AD&D Premium reductions for $50,000 option New coverage option of $10,000 available 13

Benefit Changes – STD Premium reduction-new rate $.65/$100 Maximum duration of benefits Injury benefits start on the 1st day of injury up to 26 weeks Illness benefits start on the 31st day of illness up to 22 weeks; or Illness benefits start on the 61st day of illness up to 18 weeks Benefit payment offset by 100% of employee paid sick, annual and donated leave after the waiting period is exhausted Change will only affect STD claims with dates of disability on or after January 1, 2015 15

BeWell Wellness Programs and Services The BeWell Wellness Benefit includes the following: • Mini health screenings • Mobile mammography and prostate cancer screenings • Seasonal flu shots (in progress now) • EAP services through ComPsych for contracted agencies • Tobacco Cessations Program • Quarterly Health Initiatives with resource materials and educational information Please visit www.benefitoptions.az.gov/wellness for a complete listing of services and schedule of events. 18

BeWell Health Impact Program-NEW for 2014 Program Enhancement for Plan Year 2015 • The BeWell Health Impact Program (HIP) is a wellness enhancement to the current State of Arizona Benefit Options Plan, effective October 1, 2014-September 30, 2015. • Specifically, HIP is designed as an incentive based employee wellness program for benefits eligibleState of Arizona employees. Employees who successfully complete the program by reaching 500 activity points may be eligible to receive up to $200. • Participants will accumulate points by engaging in a variety of wellness programs and activities, then self-reporting their progress in the Mayo Clinic Healthy Living online portal. 19

BeWell Health Impact Program-NEW for 2014 Program Tools The Mayo Clinic Healthy Living Online Portal will provide the health assessment tool and tracking system as a component of HIP: • The online portal will serve as the database for all employee self-reported health activities • Verification of activities will also be a function of the online portal • Only active, benefits eligible employees will have access to the portal • Healthy Living online is available now at http://bewellstaywell.az.gov • Complete program information will be available at http://benefitoptions.az.gov/wellness 20
