  1. OPEN ENROLLMENT Website Open and Close Dates • OPENS at 12 am, October 21, 2013 • CLOSES at 5 pm November 8, 2013 • Members can go online as many times as they like, but the last election confirmed at closing time will be the election for the 2014 Plan Year. • Members should print and keep a copy of the confirmation page.

State Health Benefit Plan2014 Plan Vendors • UHC and Cigna will no longer be offered and their contracts end December 31, 2013. • Blue Cross Blue Shield of Georgia has been awarded the state contract beginning January 1, 2014. • Express Scripts will be our new Pharmacy Benefit Manager beginning January 1, 2014. • Healthways will be the new Wellness Vendor beginning January 1, 2014. • Each covered family member will receive their own ID card which will be valid for all three vendors (BCBS, Express Scripts, and Healthways).

  3. 2014 Plan Options • Members will choose from three levels of benefit coverage: • HRA Gold • HRA Silver • HRA Bronze • The above plans are HRA plans. • The HMO and HDHP plans have been eliminated.

Health Reimbursement Account BENEFITS: • Initial HRA credits vary depending on the option/tier chosen • Any remaining HRA credits form 2013 will be rolled over to 2014 plan INCENTIVES: • 2014 Wellness Incentives are increasing up toa maximum of $480 for the member plus up to an additional $480 for covered spouse. • Members/Spouses who completed their wellness requirements in 2013, the $240 will be credited to their HRA account on January 1, 2014.

Transition of Care • If a member is currently receiving or anticipates the need to continue certain covered services and/or medical treatments beyond 2013, and the provider is not participating in the BCBSGa provider network he/she may request transition of care services. Transition of care services allows a member to continue to obtain certain ongoing services and/or treatments from his/her out-of-network provider for an approved period of time • The member will need to contact BCBSGa customer service no later than December 31, 2013 to obtain specific instructions for requesting transition of care services. • Blue Cross Blue Shield customer service: 1-855-641-4862 and www.bcbsga.com/shbp
