Benefit Philosophy Genomic Health's benefits program is designed to: Protect you and your family against catastrophic financial loss Emphasize prevention and health maintenance Promote quality of life Allow the opportunity and encourage the accumulation of retirement savings Support excellence in our workforce Provide flexibility and choice to meet various employee needs

Benefits Eligibility You are eligible to enroll in benefits if you are a full-time employee working at least 30 hours per week You have 31 days from your hire date to make your benefit elections No default coverage Must wait until next open enrollment if benefits later desired Your benefits are effective as of your hire date After your initial enrollment you can only make changes if you have a “qualifying event” or during the next open enrollment in April Each year Open Enrollment is held in April with changes effective May 1st

Eligible Dependents You may also enroll your eligible dependents in the benefits program, including: your legal spouse your qualified domestic partner your unmarried children to age 19, or until age 25 if they are a full-time student at an accredited school and an IRS dependent Your children include: natural and adopted children stepchildren who are IRS dependents and who live with you in a parent-child relationship child(ren) of your qualified domestic partner any other children who are IRS dependents for whom you are the legal guardian or for whom you are required to provide coverage as the result of a qualified medical child support order

Qualifying Event The benefit choices you make when you enroll will remain in place for the entire plan year unless you experience a “qualifying event” or “change in family status” such as: The addition of a dependent through birth, adoption or marriage The loss of a dependent through divorce or death, or if your child reaches the maximum age limit for coverage The gain or loss of your dependent(s) coverage under another health plan If a qualifying event occurs in your life, you must adjust your benefits election within 31 days of the change by making a change in BeneTrac

Choosing the Right Medical Plan HMO (available only in California) Most benefits paid at 100% after $10 co-pay for office visits Care directed by your primary care physician Low out-of-pocket cost with little or no paperwork PPO or Blue Card PPO (only outside California) Co-pay of $10 for office visits Benefits paid at 90% or 70% depending on provider network status Self-directed care = freedom of choice Higher out-of-pocket cost and may require some paperwork

How the HSA Plan Works Blue Cross Lumenos HSA – PPO High deductible: employee only $2,500 / family $5,000 Must also meet IRS eligibility requirements Meet the deductible, then benefits paid at 100% or 70% depending on provider network status Preventive care covered at 100% in-network with no deductible BUT all other services including Rx subject to deductible GHI funds half of the deductible over 12 months You can make additional pre-tax contributions to your account Annual federal limits = employee only $3,050 / family $6,150 Catch up contribution of up to $1,000 available for over age 50 Calendar year deductible so review your contribution amounts at the beginning of each year

  13. IRS Eligibility and Rules for the HSA? You can only be covered by a HSA-compatible, high deductible plan if you have no additional coverage You cannot be enrolled in Medicare or, if 65, must waive Medicare You cannot be claimed as a dependent on another person’s tax return Employees in HSA can not participate in Health Care FSA Employees whose spouse is enrolled in a Health Care FSA can not participate in an HSA Dependent care FSA is separate from the healthcare FSA and does not conflict with the HSA

How to Use Your Direct Dental Plan To use your Direct Dental Plan, present your ID card to your dental office Direct Dental will fax a Complete Benefit Summary to your dentist upon request Your dentist may mail or fax claim forms and receive payment directly from Direct Dental To help stretch your benefit dollars, the nation-wide network of PPO dentists offer an average discount of 20% to 30% on dental fees Visit www.directdentalplans.com and select the DHA link to find network dentists in your area!

Flexible Spending Account Administered by ADP Plan Year is not on the Calendar Year May 1st – April 30th Health Care Reimbursement Account Pre-tax deductions to pay for out-of-pocket medical expenses $5,000 annual maximum Claim can be submitted as soon as expense is incurred Dependent Care Reimbursement Account Pre-tax deductions to pay for dependent child and disabled adult daycare $5,000 annual maximum Claim can be submitted once expense is incurred AND funds deposited

  19. How Does a Flexible Spending Account Work? Calculate expected expenses for the Plan Year (not the Calendar Year) Claims must be incurred during Plan Year while actively participating ADP offers a debit card 90 days to submit claims after end of plan year Unused funds will be forfeited to plan expenses Use it or lose it is an IRS rule

Higher income replacement, up to 75% of your total income Tax free benefit You own it – this policy is portable 100% Employee Paid • It’s voluntary coverage you pay with post-tax dollars so benefits are tax free under current tax laws. • It’s offered at a discounted rate because you're buying through Genomic Health, Inc. • It supplements your group long term disability coverage and can protect a higher percentage of your income. • During this enrollment, no medical exam is required for eligible employees who are actively at work. You will be asked a few eligibility questions. • The individual disability insurance policy belongs to you, even if you change jobs.

  27. 401(k) Administered by Securian You are eligible 1 month after your hire date You will be automatically enrolled at 2% unless you log in and opt out Company match of $1,000 per year on a per pay period basis in 24 installments up to a maximum of $41.67 per pay period The Genomic Health match is in addition to the IRS limits. Therefore, in 2010, if you were to contribute the IRS max of $16,500 to the plan and GHI contributes a match of $1,000, you would have $17,500 in your account, tax deferred, by end of December 2010 Log in to www.securianretirementcenter.com to opt out or make changes to your contribution rate

Training Assignments You will be receive your new hire trainings assignment by e-mail. It is required that you complete all assigned online Compliance, HIPAA, Health & Safety and Sexual Harassment training within 30 days from the date assigned. HIPAA & Compliance Certifications Return certifications to HR upon completion of your training assignments. These documents are available on the intranet and are also embedded in your Compliance and HIPAA trainings.
