  4. Transition Plan When an individual is no longer in housing the MCO/LME should immediately send notification within 24 hours to the Community@dhhs.nc.gov (Community Mailbox) DAAS staff will immediately notify Jessica who will notify the Reviewer.

  8. Services Update • MCOs must ensure ACT and SE (mh/sa) providers are in full compliance with the new service definitions regardless of fidelity screening. • New Definition Live Dates • ACT-August 1 2013 • SE- May 2 2013 (hold harmless ends September 1)

  9. Thinking It Through The “tone” of In-Reach: To support individuals in understanding their options and to be a non-judgmental resource in thinking it through Thinking It Through Document – a support for Dialogue.
