  1. Low Income Subsidy (LIS) Medicare Prescription Drug Extra Help Online Application

  2. First things first… Make sure the beneficiary knows you are not enrolling them in a Medicare Part D Plan!!

Who should not complete an Extra Help application? • The beneficiary receives Medicaid with prescription drug coverage and they have Medicare, or • The state pays for their Medicare premiums (Medicare Savings Program), or • They receive Supplemental Security Income (SSI) payments and have Medicare. If the beneficiary is in one of the 3 groups above, they are automatically eligible for the Extra Help and do not need to apply.

  4. What information will you need? Social Security needs information about the beneficiary’s (and spouse if married and living together) income and resources. Documents that may help you prepare their application includes: • Social Security card; • Bank account statements, including checking, savings, and certificates of deposit; • Individual Retirement Accounts (IRAs), stocks, bonds, savings bonds, mutual funds, other investment statements; • Tax returns; • Payroll slips; and • The most recent Social Security benefits award letters or statements for Railroad Retirement benefits, Veterans benefits, pensions and annuities. If these documents are not available, provide their best estimate so that Social Security can determine whether they are likely to qualify for extra help with their prescription drug costs. The documents will not have to be submitted unless contacted by a Social Security representative.

  6. And then… Keep on filling in the application. It’s just that easy! It will take you ten minutes at the most! And don’t forget to ask: “Who do you know that might be in this same situation and I can help?” Follow up: • Remember to call your client after 14-21 days to see if they have received any correspondence from Social Security. • If the beneficiary is eligible for Extra Help and they have received the approval letter from Social Security, they are now eligible for an SEP and you can discuss switching Part D plans, if it’s in their best interest.
