(The Risk Monitor is not a part of the Imperative package but is available at an additional cost)

Background Information • Nursing Homes: Fee is based on licensed beds that are not laid away • Registered Housing with Services: Fee is based on units • Independent Living: $3.75 per tenant survey and $2.95 per employee survey • Minimum Fee: $75 per month • Can cover multiple entities of an organization if enrolled simultaneously • If not enrolled simultaneously, applies separately
