  6. Fee is $10 per person

  7. Forms – take note • Rank as of June 1, 2008 • “Current” not accepted for immunization history • Do not attach copy of shot record. Just record date. • Ratio – 4:1 (youth: adult) • Signature required on BOTH front and back of form. • Medication form must be filled out and given to nurse with meds at time of camp.

  8. Pack turn in dates • Monday, March 10th from 7:00p.m. – 8:30 p.m. at Fort Bend County Library – Sugar Land Branch, 550 Eldridge Road • Thursday, March 20 from 7:00p.m. – 8:30p.m. at Fort Bend County Library First Colony Branch, 2121 Austin Parkway (Tentative Location) • Thursday, April 8th from 7:00 pm to 8:00 pm at Round Table

  9. What to turn in? • All COMPLETED** forms. • Check to cover fees-made out to SHAC • CD with registration info loaded *Additional check to cover cost of registering new adults.
