Self Assessment – Required by GS-R-3 • Written Objective: To ensure that the Safety, Health and Environmental performance of facilities are subject to periodic self-assessment and independent review and that any conditions limiting the Authority to Operate (ATO) are identified. • Wider Objective: • Supports good Safety Culture by reinforcing: • Ownership / Leadership of plant, systems and staff. • Questioning Attitude, • Continual Improvement • Objectives achieved by the Senior Plant Managers’ review of: • Their SHEQ Management System. • Their plant Safety Case and supporting rules, instructions, etc. • Their SHEQ performance. • Number of and competence of their Staff. • The physical state of their Plant. • Actively seeking improvements to their arrangements.

  4. SELF ASSESSMENT ATO Renewals • Review of all aspects of ATO Facilities • Emphasis on self-monitoring to produce annual report • Evidence reviewed by Independent Assessor • Review of safety documentation, waste arisings, environmental discharges, maintenance, records etc.

  5. SELF ASSESSMENT • ATOs are issued initially following production and approval of safety cases and implementation plans, ATOs are not issued until safety controls and safety management arrangements to affect the safety case are in place. • ATOs include the identification of a programme of self-assessment activities to provide assurance that the safety, health and environmental (SHE) risks are being effectively monitored and controlled for the ATO period.

  6. SELF ASSESSMENT • Aim to commence Self Assessment Report (SAR) review ~3 months prior to ATO Certificate expiry date or as soon as possible after the ATO reporting review period. • Pre-meeting. The ATO Holder and/or representative to agree the menu content of their SAR with an Independent Assessment representative.

  7. SELF ASSESSMENT • The SAR is prepared in a typical report style with flowing text (i.e. not in constant tabular form). • Appropriate graphical and tabular information may be included to assist clarity.

  8. SELF ASSESSMENT • In production of the SAR, the author is encouraged to self-generate (where appropriate), Actions/Recommendations in the Report. The Actions will thereafter be entered, by the local plant management, in their self-selected Action Tracking system. • If there are significant issues the Assessor can raise formal Corrective Actions. • In all cases actions are checked for progress during the following review period.

SELF ASSESSMENT Independent Assessor: • Checks compliance with the required/stipulated Self Assessment Report (SAR) content list. • Checks compliance with supporting evidence supplied in the “Information Pack”. • Makes a random selection of the related topics to further check validity e.g. risk assessments, operating instructions, event reports etc. • Contacts Facility Advisors (Radiological Protection, Industrial Safety Environmental) to establish any issues they may have for the facility.

  10. SELF ASSESSMENT • Independent Assessor physically tours the facility ATO (inside and out) to establish if the general condition of the facility maps the tone and contents of the SAR. The tour to includes discussions with facility based personnel e.g. operators and supervisors.

  11. SELF ASSESSMENT • The Self Assessment report formally records and reports the documented review undertaken. • The documented review is passed to the facility ATO Holder to arrange the comments and observations to be formally addressed • On return of the documented review from the ATO Holder the review is closed out and recommendations made on renewing the ATO. • The fully completed electronic documented Close-Out report is returned to the ATO Holder for including in the Self Assessment Report.
