Why Joint Strategic Needs Assessment (JSNA) on Homelessness 2011 identified need for common approach to support people from homelessness to their most appropriate level of settled independent housing • 22% of service users in non-emergency accommodation for 2years + • 12% of service users resident in non-emergency accommodation for 5 years +
Aim • Build on recognised success of: • Pathways to Independence • Lead Practitioner roles • Gateway • YHN Pathways ASWs • Develop common processes for assessing and reporting readiness for move on • Integrate the referral and monitoring processes of the Pathways to Independence and Advice and Support workers with the Gateway • Agreeing a consensual approach to making the best use of the available housing and support
Aim • Key will be: • Introduction of common approach to assessing capacity to move to independence • Expectation at 6 month point that clients’ readiness for move on will have been assessed • Reporting and monitoring readiness to move on • Have identified 3 main mechanisms to achieve this
1. YCH eligibility test • Using YCH eligibility test as a tool for assessing readiness for move on • Application to be made within 2 weeks • Addressing reasons for ineligibility
2. Gateway Assessment Assessment to be made on clients Gateway record as to their readiness to move. • Client likely to require ongoing intensive support in long term accommodation • Client is assessed as requiring further support and intervention from provider or other services before being ready. • Client is ready to move to independence and maintain a tenancy with or without ongoing support. Assessment to be updated when support plan reviewed
3. Move On Panel • Monthly meeting chaired by Lead Practitioner – Complex Needs • Attended by Providers and staff from YHN Pathways Advice and Support Team • Opportunity to assess those not moving on • Providers able to seek assistance with difficult cases
Summary • Use Gateway to record progress • Application to YCH (soon to be Tyne and Wear Homes) at early stage to determine eligibility • Assessment made on Gateway record as to clients readiness to move • Monthly Move On Panel to monitor and assist with the process
