NY Connects • Information available by phone, online, or in person. • Connect to long term services and supports regardless of diagnosis, age or how you will pay for services. • Receive guidance and coordination as you go through assessment and eligibility processes. • Help with completing applications and enrollment in public assistance benefits, such as Medicaid. • Call 315-785-5081 NY Connects is your trusted place to go for free, unbiased information about long term services and supports in New York State for people of all ages or with any type of disability. Long term services and supports include medical and non-medical services a person needs to improve or maintain health and independence. They may be provided in a nursing home, the individual’s home or other community-based settings.

EISEP • Must be 60 years or older • Not on Medicaid • Able to be maintained safely at home • Waiting list for in home services and PERS • Income guidelines • $1508 for single • $2032 for couple • Cost share for anyone over income Expanded In Home Services for the Elderly Is a coordinated package of non-medical services and supports developed from an in home assessment to assist older adults who need help with Activities of Daily Living, e.g., dressing, bathing, personal care and Instrumental Activities of Daily Living, e.g., housekeeping, shopping, cooking. All enrollees receive case management.

Senior Nutrition Program • Menus are planned by a Registered Dietician to meet 33% of the specific Dietary Reference Intakes (DRI) for older adults. •  No accommodations are made for special diets, although salt is not added in cooking. Our Senior Nutrition Program Dietician is available to answer questions and provide nutrition counseling upon request. Please call 785-3191 to talk to the Dietitian. • We only ask for a suggested donation of $3.50 for each meal served at a congregate site and $3.50 for a home-delivered meal. Please remember that you are asked to donate whatever you can afford. No eligible senior will be denied a meal due to the inability to donation. Guests under age 60 must pay a $6.46 fee. • Participant donations greatly assist in providing supplemental funding for our Senior Nutrition Program. • Congregate participants may donate daily, weekly or monthly by cash or check at the meal site. Home-delivered meals clients will receive a monthly donation letter at their home and donations should be made by check to the Jefferson CountyTreasurer. Home Delivered Meals – hot noontime meals delivered to eligible seniors and their caregivers through out the county M-F Congregate Meals - Dining-In Program providing a nutritious noontime meal and an opportunity for socialization, recreation, and health information at meal sites throughout Jefferson County. Seniors 60 and older, their spouses and caregivers, regardless of income, are welcome to participate.

Benefit Counseling • Medicaid – helps pay for medical care, homecare and nursing home costs • SNAP – Food Stamps – helps pay for food • HEAP – Home Energy Assistance Program – helps to heat your home • Medicare Savings Program – helps to pay your Medicare B premiums ($135.50/month) • EPIC – Elderly Pharmaceutical Insurance Coverage – helps pay for prescriptions • Mybenefits.ny.gov Basic screening by NY Connects staff to determining eligibility for income based programs.

There are other important preventive benefits - like recommended cancer screening to help detect disease early, when it's most treatable.

Medicare • Medicare Part A - hospital insurance covers inpatient hospital care, skilled nursing facility, hospice, lab tests, surgery, home health care. • Medicare Part B – medical insurance covers doctor and other health care providers' services, durable medical equipment, home health care, and some preventive services. • Medicare Part D – prescription drug coverage. Health insurance program for: People age 65 or older. People under age 65 with certain disabilities. People of all ages with End-Stage Renal Disease (permanent kidney failure requiring dialysis or a kidney transplant).

Health Insurance Information, Counseling and Assistance - HIICAP • HIICAP provides free, accurate and objective information, counseling, assistance and advocacy on Medicare, private health insurance, and related health coverage plans. • HIICAP helps people with Medicare, their representatives, or persons soon to be eligible for Medicare.
