API RP 2D JSA Work Team • APPENDIX B – Commentary on Job Safety Analysis • The safety and success of crane and lifting operations begins with the development of a Job Safety Analysis (JSA) that recognizes hazards and eliminates or lowers the identified risk to an acceptable level. By the development, implementation and review of a JSA process for crane or lifting activities, crane owners should be able to conduct these operations in a safe and sound manner by following this recommended practice. The following are the recommended methods and minimum guidelines that should be considered in determining the need for and in conducting a JSA as part of a safe and successful crane or lifting operation.

API RP 2D JSA Work Team Safe offshore crane operations are determined by the following factors: • Mechanical Integrity of the Crane • Lifting Capability of the Crane • Proper Lift Planning • Proper Training of Personnel Involved • Proper Communication • Identification of Hazards Associated with a Lift • Elimination or Mitigation of Hazards Identified Through the JSA Process • Review and Improvement of Previously Conducted JSA’s

  4. API RP 2D JSA Work Team a. The JSA process should identify the hazards that may be encountered throughout the lifting operation or crane activity. Hazards may include, but not be limited to: • Environmental conditions • Side-loading • Shock-loading • Loss of communication • Slip/trip/fall hazards • Inaccurate weights of equipment • Incorrect use of loose gear • Inexperienced personnel • Open holes • Emergency escape route may become blocked or inaccessible • Contingency planning

  5. API RP 2D JSA Work Team b. The JSA process should identify measures taken to eliminate or mitigate any hazard or safety concern identified through the JSA process. c. The JSA process should identify the proper equipment needed to make the lift successful. Proper equipment may include, but not be limited to, the following: • Well maintained crane • Loose gear (slings, shackles, spreader bars, etc...) • Mode of communication • Tag-lines

  6. API RP 2D JSA Work Team d. The JSA process should include an alternate emergency procedure should unforeseen circumstances jeopardize the safety or success of the lifting operation. e. The JSA process should identify the physical requirements and/or limitations, environmental conditions, personal protective equipment, and safety factors relating to the specific lifting operation.

  7. API RP 2D JSA Work Team f. The JSA process should be conducted by a designated person in charge of the lifting operation . g. The JSA process should identify if the lift to be made is static/onboard or dynamic/offboard.

  8. API RP 2D JSA Work Team h. The JSA process should identify the equipment to be lifted the weight of the lift and sequence of lifts. i. The JSA process should identify if any lift will be made over a hazardous areas such as a well bay, process equipment or living quarters. j. The JSA process should include participation of all personnel involved with, or affected by, the lifting operation.

  9. API RP 2D JSA Work Team k. The JSA process should ensure that all personnel involved with the lifting operation know their role and responsibilities for the lifting operation. l. The JSA process should ensure that all personnel involved with, or affected by, the lifting operation are aware of their “Stop Work Authority” and their obligation to exercise the authority should they see a potential hazard that has not been identified or mitigated.

  10. API RP 2D JSA Work Team m. The JSA process should be performed in conjunction with, and compared to, any procedure or lifting plan if such procedure or plan is available to ensure all hazards are identified and remediated.
