Contiuity of medicalrecords(informationalcontinuity) • The degree of access of any service provider following a patient to thecomplete information aboutpatient's previous illnesses and applications to the health care system. • Qualified health care requires the continuity of the records about patients' medical problems over time, location and episodes of illness.

Interdisciplinarycontinuity Familyphysician; • Does not onlycare for a medical problem or organ system • Playa role in the management of their patients' medical, social and family problems • Ensures continuity of care of the patients with multiple problems by coordinating other disciplines
