REACH II Intervention Goals: The overall goals of the intervention are threefold: 1) to identify and reduce modifiable risk factors and negative consequences of caregiving, 2) to enhance quality of care of the care recipient, and 3) to enhance the well-being of caregivers. • Objective 1 • Increase knowledge of caregivers about: • Disease and treatment options • Caregiving: duties,demands, consequences • Available resources • Safety and physical environment • Health related behaviors communication • Objective 3 • Increase caregivers’ skill to: • Manage ADL/IADL problems • Manage behavior problems • Enhance safety • Manage emergency situations • Communicate effectively with care recipient, family members, health care providers • Objective 2 • Enhance support to caregivers • Emotional and social support • Support with instrumental activities • Objective 4 • Enhance caregiver well-being • Physical well-being • Emotional well- being Risk Appraisal: Identify and prioritize caregiver problems/risks • Intervention Strategies • Information and referral • Provide educational materials about AD, problem behaviors, nutrition, etc. • Provide access to available resources through the resource guide on CTIS. • Education/didactic instruction • Provide access to educational information through education component of the CTIS. • Intervention Strategies • Information and referral • Provide access to available resources through the resource guide of the CTIS. • Education/didactic instruction • Reinforce strategies/skills related to accessing formal/informal support, communication in support groups and use of education component of the CTIS. • Support • Caregiver support groups. • Intervention Strategies • Information and referral • Educational materials on topics related to behavior management, etc. • Education/didactic instruction • Didactic instruction on topics such as incontinence, wandering, etc. • Reinforce strategies/skills using the educational component of the CTIS. • Active (hands-on) learning • Teach and actively practice techniques to manage behavior problems; enhance communication, etc. • Intervention Strategies • Information and referral • Educational materials on topics related to relaxation, stress management, etc. • Provide access to available resources (e.g., sleep clinics) via the CTIS. • Education/didactic instruction • Provide didactic instruction on topics such as stress management, socialization, healthy living. • Reinforce strategies/skills using the education component of the CTIS. • Active (hands-on) learning • Reach and actively practice relaxation and stress management techniques. • Support • Use reminder function of the CTIS (e.g., medication; MD appointments. • CG respite function on the CTIS.
