Three Broad Roles of CCACs • Information about and referral to other community services. • Provision of publicly funded health services in the Client’s home, clinic and/or school. • Determine eligibility and authorization admissions to Long-Term Homes and some complex continuing care facilities based on assessment of need and eligibility. Legislative Framework • Governed by Home Care and Community Services Act • Community Care Access Corporations Act, 2001 • Bill 36 LHIN Act • Board of Directors made up of individuals from across the North East who meet once a month at various locations (Current Chair: Ron Farrell) North East CCAC
North East Facts and Stats North East CCAC • The North East CCAC is one of 14 CCACs in Ontario • Formally established on January 1, 2007 • An amalgamation of the former: • Algoma, Cochrane, Manitoulin-Sudbury, Near North, Temiskaming, and West Parry Sound (WPSHC) CCAC s. • Six branches: • Kirkland Lake, North Bay, Parry Sound, Sudbury, and Timmins. • Staff members are also located in a number of satellite offices including offices in community hospitals.
North East CCAC Facts and Stats North East CCAC • The North East CCAC services approximately 16,000 people in North Eastern Ontario on any given day • Currently: • 37,625 total clients (in-home, school service, placement) • Approx. 600 employees • 240+ Case Managers • 100+ professional therapy staff • 76,279 therapy visits/year • 365, 235 nursing visits/year • 1,077,841 personal support hours/per year • 110 contracted service providers • $115 million budget 2011-2012

Service Hours Telephone access to Case Managers and Team Assistants: Monday to Sunday, 365 days/year 8:00 a.m. to 8:00 p.m. Office Hours: Monday to Friday 8:30 a.m. to 4:30 p.m. North East CCAC
Who is eligible for our services? North East CCAC • A valid Ontario Health Card • Health needs that cannot be met by a hospital/community outpatient service • A medical condition that can be adequately treated in the home, our clinic or school. • A need for at least one professional or personal support service • Need for information and assistance when applying for long-term care • ** For those individuals who do not qualify for CCAC services, we can offer Information and Referral to alternate services within the community.
Who can make a referral for service? • Referrals to the North East CCAC can be made by: • Individuals • Family members • Caregivers • Friends • Physician • Health care professional North East CCAC
Referral Process North East CCAC
Case Management North East CCAC • North East CCAC Case Managers are regulated health care professionals with specialized knowledge and skills who help assess the clients’ needs, assist them in developing a care plan and arrange for the services required. • Case Managers comprise approximately 45% of the staff at North East CCAC
Services Available and Accessed Via Case Managers: - Nursing - Placement Coordination - Personal Support - Referral to Community Services - Caregiver Respite - Physiotherapy - Occupational Therapy - Speech Therapy - Social Work - Dietetic Services North East CCAC
Who Pays for Services? • The Ontario Ministry of Health and Long-Term Care pays for services provided by the CCACs (through the North East Local Health Integration Network or LHIN). • The Ontario Ministry of Health and Long-Term Care has established upper limits on the amount of service CCACs can provide to Clients. North East CCAC
Care Plan • The Case Manager creates the care plan with the client or substitute decision maker based on assessed need, NECCAC service guidelines, and legislated service maximums • The Case Manager reassesses and monitors the service plan with input from the client and service providers and makes adjustments when necessary. • We strive to offer services in French and English North East CCAC
Service Discharge • A person remains an NE CCAC client as long as there is a service need that can be responded to as determined by the Case Manager. • A client is discharged when the Case Manager and the Client/Substitute Decision Maker determine the service is no longer warranted. North East CCAC
