On March 4, 2009, at 08:40 hours, an electrical flashover incident occurred involving a contractor seeking information about 480 KV breakers. The contractor did not obtain a work permit and attempted to remove the breaker without permission, leading to the dangerous situation. As a result, three contractor employees were injured; one sustained first-degree burns, while two suffered second-degree burns. All victims received first aid and were later taken to the hospital for further evaluation and treatment.

EXECUTIVE SUMMARY
On 03/04/2009 at 08:40 hours an electrical flashover was reported while the contractor was trying to find the information regarding 480 KV breakers.
