Clients are caught in a fragmented system, filling out paperwork at various locations without a clear understanding of their application statuses. The lack of a common network or medical records system hinders effective inter-agency communication and coordinated care. As we strive for 100% access and zero disparities, we must prioritize solutions such as community collaboration, improved application tracking, and culturally competent care to transform our vision into reality.
