  3. 2005: Projected Nursing Shortage Demanded Action Projected shortage of 35,300 RNs in TN by the year 2020!

  4. Leading the Charge … Fall 2005 Convened Deans/Directors and CNOs in Nashville to brainstorm the barriers to preparing more nurses! Tennessee Center for Nursing responded to the looming shortage by gathering data and searching for solutions! Ann Duncan Executive Dire3ctor TCN

  5. It wasn’t as simple as clinical placements …

Barriers to Increasing Enrollment & Placing Students … • Knowledge about potential open placements. • Time required to secure placements – both in the school and facility. • Redundant clinical orientation content for students/faculty. • Time required in facility to orient multiple student groups to clinical facilities. • Time to secure new clinical facility contracts.

  7. Nashville Pilot: Spring 2006 • Grant Money for startup • 3 Hospitals & 4 Schools • Non-Profit, Profit, Metro & Community • Private/State • Used StudentMax to manage placements • Used Online Orientation for students & faculty

Tennessee Clinical Placement SystemSM • Adopted the StudentMax software for clinical placement management. • Maintained long-standing relationships with STANDING • Defined a placement as one day/one shift/one unit • Identified existing placements and all open • Created Group, Precepted, and Observation placements

TCPSSM Online Orientation • Created online content that was common to all facilities based upon OSHA, JC, CDC, HIPAA, and other regulatory mandates. • Developed a quiz to monitor student outcomes and provide documentation of successful completion. • Worked with facilities to add facility-specific content

  10. Growth of TCPSSM • 2006: Tennessee Clinical Placement Partnership & TCPSSM active in Middle TN • Additional Regional Partnerships formed and govern themselves • Promoted TCPSSM is a TOOL! • Limited policies • Flexible for regional needs whenever possible

  11. TCPSSM Statewide Activity • Middle (Nashville) • West (Memphis) • Southeast (Chattanooga) • MidWest (Jackson to Dyersburg) • East (Knoxville) • Not in NorthEast (TriCities) • Additional TN partners from MS, AR, GA, & KY

  12. Framework for state-wide deployment • Began with nursing, now some allied health • No region is the same yet each can benefit from the strengths of other regions. • Established primary contacts for service/academia in each region – service usually drives regional implementations.

  14. Clinical Placements Clinical Placement Opportunities were being missed simply because we didn’t know they existed!

  15. During the pilot phase in Middle TN, 43 new open placements were identified in just 4 clinical facilities.

  16. Today in five regions of TN there are 6788OPEN placements for fall 2010!

  17. Prior to implementing TCPS, the average time spent in both facilities and schools to initiate and confirm a clinical placement was 16 days.

  18. Today clinical placements are requested online & usually confirmed within 5 working days! * *Does not include contract negotiations!

  19. Placement ROI: Less time arranging clinical placements results in more time available for other activities! Posting ALL OPEN placements has the potential to bring more students into the facility and improve your workforce pipeline. Facilities may open/close as needed.

  20. Online Orientation Orientation for students required Facility Educators to spend an average of 14 hours per clinical group per term.

  21. Individual orientations for precepted students where common. Allied Health student orientation was difficult to provide consistent orientation.

  22. Students were spending 3-4 clinical days in orientation each term as they were assigned to different clinical facilities.

Orientation ROI • Hourly rate of educator/staff involved in orientation • Total of prep/presentation hours • Number of student groups/individual students coming to facility or involved in clinical • Facility Example: $25* x 14hours x 12 orientations/year = >$4,200! • School Example: $25* x 14 hours x 4 orientations/AY = >$1400 * Base rate without benefits

TCPSSM Continues to Grow! • 2008-09: Additional States using TCPSSM solutions • 2010: TCN divested of TCPSSM to Cyber Healthcare Solutions, LLC & became TOTAL Clinical Placement SystemSMoutside of TN • 2010: 3 new Schools of Nursing Opening partially based upon availability of clinical placements identified through TCPS!

Overall Facility Outcomes • Increased Capacity/Pipeline – additional/new student groups are in facilities. • Potential Increased Quality for New Hires – students have already been in the facility which can improve “best fit” for individual and facility. • Time and Cost Savings – facility staff use the online environment to duplicate their time for ALL student placements.

Overall School Outcomes • Increased Capacity – access to additional clinical placement opportunities. • Reduced Classroom Orientation Time – students have opportunity to begin bedside experiences sooner in the term. • Reduced Redundancy in Orientation Requirements – students complete general orientation once per academic year – not multiple times a semester.

  29. TCPSSM TN Business Model • TN facilities/schools join the partnership within their region. • Partnership is based upon the academic year calendar. • Fees are based upon average daily census and student headcounts to yield a sliding scale fee structure for both TN & Non-TN partners.

Orientation Outcomes … • Students only complete general orientation once per academic year. • Completion time averages <3 hours for General Orientation! • Comments have been favorable from both students, faculty, and facilities! • >5500 students/faculty in TN area participated in AY 2009-2010. • Fall 2011 we expect over 15,000!
