  2. Why is Psoriasis difficult to treat? • Psoriasis is a very complex immunological disease which is not fully understood. Multiple parameters play a role in development of this disease and this varies from patient to patient. • In order to treat the patient of psoriasis, one has to investigate all the above stated areas and plan the treatment accordingly. Most of the systems of medicine are not equipped to address all the complex causes of psoriasis in totality to treat this disease. • Homeopathy is somewhere close to treating the disease in totality; however, not always a sure shot therapy. The homeopathic approach involves the understanding the complete case analysis of the patient, understanding the case in terms of totality of symptoms and understanding the complex mindset and emotional state of the patient. After deeply studying the above criteria, homeopathic medicine is selected and given to the patient. And talking about scalp psoriasis, it is a common variant among the various types of psoriasis. • It can vary from very mild thin flakes sometimes mistaken as dandruff, to very severe thick flakes which are associated with bleeding and severe scratching. This may gradually spread over to the forehead, behind ears or nape of neck. Caring for scalp psoriasis • Moisturizing the scalp– you can start application of essential oils or gels (like Alovera) to soften the skin which has become harsh and rough. • Be very gentle when applying any cream. Features to distinguish Nail psoriasis vs Fungus: • Psoriasis of the nails can be triggered following any trauma or injury to the nails (koebner’s phenomenon) as seen in psoriasis of the skin. • Fungus of the nails or onchyomycosis is caused by a fungus causing infection. There are some factors which predispose one to contract fungal infection of the nails. The most important factor which favors fungus is warm climate. Fungus grows well in a warm and humid climate. Hence it is mostly found in people who sweat a lot, individuals who love swimming and do not dry their body properly (Especially the spaces between the toes). • The two can be differentiated by the appearance, as psoriasis has a classical yellow nail pitting (indentation) which is not found in any fungal infection. • The yellow pitting in psoriasis and hyper pigmentation (dark discolouration) in case of fungus can be the strong differentiating points. • Fungal infection usually spreads from the toes (Tenia pedis or athletes foot), to the inter-digital spaces followed by spread to the nails. It starts with a small yellow dot beneath the nail. In fungus infection there is a foul odor with dull appearance of the nails. • Psoriatic nails gradually tend to get separated from the nail bed (oncholysis) whereas the fungal nails just get deformed. There is a chalky appearance under the nails which lifts up the nails which further causes pain and tenderness.
