Understanding Hydrocephalus An Overview by Dr. Ishant Rege – Consultant Brain & Spine Surgeon What is Hydrocephalus? Hydrocephalus is a medical condition characterized by an abnormal accumulation of cerebrospinal fluid (CSF) within the ventricles (fluid-containing cavities) of the brain. This excessive buildup results in increased intracranial pressure, which can damage delicate brain tissues and impair brain function. CSF normally flows through the brain's ventricular system and is absorbed into the bloodstream. It acts as a cushion, delivering nutrients and removing waste. When this balance is disrupted—due to overproduction, obstruction, or poor absorption—fluid builds up, leading to hydrocephalus.
Symptoms to Watch For The symptoms of hydrocephalus vary based on age, disease progression, and underlying cause. Here are key warning signs: In Infants: Enlarged head size: Noticeable increase in head circumference. Bulging fontanel (soft spot) on the head. Irritability and excessive sleepiness. Poor feeding and vomiting. Downward gaze of the eyes (sunsetting eyes). In Older Children and Adults: Persistent or severe headaches. Nausea and vomiting. Blurred or double vision. Difficulties with balance and coordination. Cognitive difficulties, including memory issues and slowed thinking. Urinary incontinence (in some cases). Early detection and treatment are critical to prevent long-term neurological damage Causes of Hydrocephalus Hydrocephalus can be congenital (present at birth) or acquired (developed later due to injury, infection, tumor, or bleeding). Common causes include: Birth defects like spina bifida Brain hemorrhage, especially in premature infants Infections such as meningitis or encephalitis Head injuries or brain trauma Brain tumors or cysts blocking CSF flow
Diagnosis Hydrocephalus is diagnosed using neuroimaging techniques: Ultrasound (especially in infants) MRI (Magnetic Resonance Imaging) CT (Computed Tomography) scans These tests help visualize the enlarged ventricles and assess the underlying cause. Treatment Options The primary goal of treatment is to relieve pressure on the brain and restore normal CSF flow. Treatment options include: 1. Shunt System Installation A ventriculoperitoneal (VP) shunt is the most common treatment. It diverts excess CSF from the brain to another part of the body (usually the abdomen), where it can be absorbed. Shunt systems require monitoring and occasional revision. 2. Endoscopic Third Ventriculostomy (ETV) A minimally invasive procedure. A small hole is made in the floor of the third ventricle to allow CSF to flow around the obstruction. 3. Medications (in rare cases) Used temporarily to reduce CSF production. Not a long-term solution.
