  1. FTE SELF-REVIEWAttendance Services • 07-08 is an audit school year • The Auditor’s General Office will review students’ records for compliancy with the information reported during the FTE survey process. • Exceptions will result in funding reductions in the various areas: Per student funding allocation (Attendance), SPED, ESOL, OJT (Vocational) and Teachers’ Certification. This could potentially impact the district’s fiscal budget. • In preparation for the State Audit, schools must conduct an FTE Self-Review.

Elementary and Secondary Schools Surveys October Survey 10/8 – 12/07 February Survey 2/4 – 8/08 Attendance Eligibility Windows October 9/28/07 – 10/12/07 February 1/24/08 – 2/8/08 • Review and verify funded students’ information reported for the October and February surveys with current documentation; Ensure information was accurately reported and transmitted to Department of Education. • The link below will access you to checklists and forms outlined on the FTE Self-Review Resource Listing. COMMON AUDIT EXCEPTIONS • Errors identified during the Self-Review must be corrected by submitting either an Attendance or DECO correction form(s) to 9028 – Attendance Services – FTE Office rm. 14 FTE COMMON AUDIT EXCEPTIONS • Student’s documents may be missing or have incomplete information from each area: ATTENDANCE • (1)Student funded for the survey, but did not meet attendance eligibility. CERTIFICATION • (1) Out-of-Field teachers did not have a board approved waiver (2) Parent did not receive teacher out-of-field letter (3) Missing copies of teachers’ certification or therapists’ license

COMMON AUDIT EXCEPTIONS SPECIAL EDUCATION • (1) IEP/EP, Notice of Meeting(s), Matrix of Services (2) Student funded on an incorrect FTE Domain Rating/Line # (3) Matrix of Services added incorrectly (4) General Educational Teacher’s Statement (5) Professional Signatures, Dates (MM/DD/YY), Demographics, General information ENGLISH SPEAKERS OF OTHER LANGUAGES (1) Home Language Survey, (2) Notification Letter to Parent, (3) LEP Plan, (4) Testing Instrument (5) LEP Committee, (6) Dates ON-THE-JOB-TRAINING (OJT) (1) OJT Time Card, (2) Signatures from either Teacher, Student, or Employer, (3) Job Search Records Log, (4) Student not Employed, (5) Worked less than 15 hours per week.
