Student Teaching Preparation Student Teaching Application Card – blue • current phone and NIU e-mail address • address from where you will student teach • health considerations – limitations • past or present personal and/or professional relationships • any additional information we should know If this information is omitted and your placement has been made, we are unable to make changes.

  5. Student Teaching Preparation – cont. • complete required forms by due date • completed forms to Clinical Office, Gabel 152 • submit the following to Peggy Williams: • Current TB clearance card • Content Area Test results • Safety Module certificate • Due by May 16, 2011

  6. Student Teaching Registration Procedures Handout Read Carefully and Note Dates • provide your signature at the bottom • white page – return to clinical office • canary page - return to clinical office • pink page – save for your files • you are responsible for all content

  7. Required On-line Safety Module • See website on checklist. • Complete on-line safety module. • Print certificate of completion. • Bring certificate of completion to Clinical Office by the due date listed on the checklist.

Student Teacher Orientation • Save the week of Aug. 15-19, 2011 • check NIU e mail and watch U.S. Mail for IMPORTANT communications from Clinical Office • current contact info. with Clinical Office • 8:30 AM – noon in Altgeld Hall for all university meeting • 1:00 – 3:30 NIU supervisor • MANDATORY attendance

Notification of Placement • e-mail for initial contact/confirmation • receive placement envelope from • Sue Lovell 815.753.9034 slovell@niu.edu or • Peggy Williams 815.753.8353 pwilliams@niu.edu Once Placement is issued: • contact your cooperating teacher • complete interview/observation day visit • return your completed Confirmation Letter to the clinical office as soon as completed

  13. Some Additional Responsibilities Some schools request these in addition to NIU requesting them: • obtain unofficial grade transcripts from your “MyNIU” account • finger print check / criminal background check • health physical / current T.B. clearance

  15. Students must apply in advance for student teaching. (See the department adviser regarding the time to apply for placement.) Transportation to the student teaching site is the responsibility of the student. In addition to having completed the NIU certification program requirements, prior to student teaching the student must (a) have earned a minimum of 14 semester hours at NIU, (b) have earned 90 semester hours, and (c) make all arrangements for student teaching through the appropriate department. Students may not make their own arrangements for student teaching sites nor may they request a change once an assignment has been confirmed by the cooperating school. For additional requirements students should see the appropriate department adviser.
