How and When to Sign up? • Place $300 non-refundable deposit by check made out to CHAN YUIN LEE by Dec 18 (Mon) in my LFM folder (LFM’07s can mail me a check) • Required even if you are booking your own flights • Send me an email with your full passport name and whether you are leaving on 22-Mar, 23-Mar or booking your own • A survey will be sent out by Friday, Dec 15 requesting for your FULL NAME, PASSPORT#, DOB, CELLPHONE#, STAR ALLIANCE FREQ FLYER # • Please complete survey by Dec 18 (Mon) • Full payment due 60-days prior to departure • By Jan 15th • Credit Card allowed

  14. Next Steps • Committee to firm up logistics • Firm up company visits • Confirm seats and deposits for flight • Arrange hotels and intra-city transportation • Book KL-BKK flight • No immunizations required • Check visa requirements depending on who signs up
