  2. Why risk assess? • Monitoring H&S of placements is a legal requirement • SHU takes H&S / risk assessment of placements very seriously • YOU are in the best position to assess the risks to YOU on YOUR placement • identifying potential hazards allows risks to be minimised

  3. Hazard & Risk • By identifying a potential HAZARD, steps can be taken to minimise the RISK Hazard: something that could happen Risk: the likelihood of it happening Example • Hazard: Getting run over by a bus • Risk: low, under normal circumstances

  4. Risk Assessment Procedure: Completion of a PRA 1 form • Initial info completed by placement team (based on H&S checklist etc) • student sent partially completed form & guidelines • on starting placement, student risk assesses their specific placement • student sends PRA 1 form back to SHU • placement team complete info, check and approve form

  6. Completing the PRA1 • SHU placement team complete section 1 - using info supplied by the company and student • Student completes section 2 (including emergency arrangements), referring to guidance notes

Points to note: • Lab work involving toxic / hazardous materials will be classed as high risk • you must include details of how the risk will be minimised (training, supervision, use of SOPs, PPE etc) • Travel, environment and health/personal issues could also introduce medium or high risk areas

  9. Points to note: If areas of your placement are classed as 'high risk': • you can still proceed on the placement • you are not 'in danger' • by identifying the hazard you can minimise the risk

  10. Next steps: • Student returns PRA1 to SHU • Placement team check section 2, complete section 3 • provided there are no 'issues' with section 2, the placement risk assessment is approved • VT, placement team and student keep a copy of the form
