The current PHPR documentation poses significant challenges in usability and patient-friendliness. Key issues include an inconsistent format, excessive recommendations, and unclear expectations that contribute to longer patient visits. Lack of a matrix or template leads to unnecessary duplication and confusion. The blend of administrative and clinical protocols complicates understanding, particularly for RN providers, with training requirements and limited LHD involvement exacerbating the problem. Emergency revisions and frequent changes disrupt user experience and fail to consider the scope of practice.
