  3. To ensure the “Safety” of equipment in “Unsafe Condition” Lockout Purpose

  4. LTCT (LTTT) : 1. Lock 2. Tag 3. Clear (Test) & Try Normal Lockout Procedure (LTCT)

  5. 1. Physically disconnect all power or dangerous sources, such as electricity, compressed air, chemical gas, liquid, etc. 2. Lock sources with locks one by one 3. Keep the key for personal possession 1. LOCK

  6. 1. Fill “Reason”, “Name”, “Date” in the Tag 2. Hang (lock) the “Tag” on the lock 3. Multiple lockout requires one lock for each individual 2. TAG

  7. 1. “Clear” the remained electricity, air pressure, spring force, potency energy, etc. (Use instrument to “Test” to ensure) 2. Then “Try” to start the equipment to ensure safety 3. CLEAR (TEST) and TRY

  8. 1. Small electrical equipment with “Single” and “Only” power source 2. Only to change or replace bulb or parts 3. Within visible range 4. Do “Clear & Try” only GENERAL EXECPTION(1)

  9. 1. Damage furniture 2. “Tag” (and put away) from erroneously use GENERAL EXECPTION(2)

  10. 1. Change Fluorescent bulbs 2. Use insulated ladder 3. Within visible range 4. Turn of the switch only GENERAL EXECPTION(3)

  11. 1. Lockable only power source 2. Downstream only 3. Lock the power source without physically disconnect the power source 4. Still to do “Tag”, and “Clear & Try” GENERAL EXECPTION(4)

  12. 1. Not performing all the normal lockout procedures 2. Not included the above exceptions 3. Must have written Special Procedure 4. Only for the specified conditions SPECIAL PROCEDURES

  13. 1. Any one found unsafe condition 2. Performed by qualified person 3. Signed only by above individual 4. First lock, Last unlock KEY POINT(1)

  14. 5. Do not use pencil or correction liquid on Tag 6. The maximum tag period is one month* 7. No one can take off other’s lock* *Unless specially approved by mgmt. KEY POINT(2)

  15. 1. Only could be done by qualified person 2. To supply the power sources within lockout period 3. Only to observe and test the equipment 4. Perform the lockout again after observation TROUBLESHOOTING

  16. 1. Owner first lock, perform the lockout procedure, and last unlock 2. Other individuals put personal Lock and sign on Tag MUTILPLE LOCKS

  17. 1. Explain situation to the new person(s) 2. Ttransfer the keys 3. Sign off the tag(s) 4. New person perform the lockout procedures 5. New person sign on the tag TRANSFER RESPONSIBILITY

  18. 1. All the jobs completed 2. All other tags and locks removed 3. Power supplied and tested O.K. 4. All safety features replaced 5. Remove the Tag and back to normal use COMPLETE THE LOCKOUT

  19. 1. Operate the locked equipments without permission 2. Break the locks 3. Remove other’s tags 4. Stop the job and perform the lockout 5. Disciplinary action for violation VIOLATION PENALTY
