Focus is at scale Half way through 3 year project First year spent talking to communities and people, developing the concept Portal live on 14th November 2013 (version 2), version 3 May 2015 Already signed up 1,800 people in Highland as test users Target 13,200 by end May 2015 Target population over 50s, family, friends, carers and service providers

Flourish The Flourish service will support the Health and social care element of Living it Up, some ideas so far; VC consultations, PC/Tablet Mobile health monitoring/Telehealth/Telecare Digital postcards ( video clips) Self management tools (shared) Doctors appointments Experience guides Games/Health Apps Access to trusted health information Some services will be prototyped in Flourish - Autumn 2013.

  4. Betty, 58 yrs old with COPD, and recently moved to Mull a few months before being widowed, daughter lives in Edinburgh. Betty feels anxious and isolated and has had recent exacerbations of her COPD LIU could help her by

LIU could help Betty Connecting her to family and respiratory nurse Access pulmonary rehab at home (VC) Find out about local groups and activities, transport to help get her out of the house Find some volunteering opportunities Find out more about her condition, share information about COPD with her family Develop an online self management plan that she shares with her practice nurse and respiratory nurse Monitor her condition remotely Share her experiences about what has worked well for her Access on line tools to help reduce her anxiety Access a self management course/group by VC Connect up to forums and interest groups, peer support, self management courses Find services to support SDS Complete a telecare assessment

Recommendations Local support required to do local community engagement and promotion of Living it Up in communities to support recruitment Support required at all levels to get staff involved in LIU and signing up patients/clients people they work with Living it up needs to be embedded in local services
