Conference Bleeding in first half of pregnancy
Physical examination: A Thai female patient, young aged, good consciousness Vital signs : BT 36.8 C BP 110/70 mmHg PR 84/min RR 18/min HEENT : no pale conjunctiva, no icteric sclera CVS : normal S1 S2, no murmurs Lungs : clear Abdomen : surgical scar(Pfannensteil incision), soft, mild tenderness at suprapubic region, no guarding, no rebound tenderness, active bowel sound
Pelvic examination NIUB : normal Vagina : normal mucosa, slightly bloody discharge, cul-de-sac bulging Cervix : clean, os closed, slightly bleeding per os, cervical excitation +ve Uterus : N/S, R/V Adnexa : free, moderate tenderness at left adnexa
Problem list 1. Suprapubic pain 3. Missed period 2. Bleeding per vagina 4. Left adnexal tenderness
Criteria for expectant management 1.Decreasing serial B-hCG levels. 2.Tubal pregnancies only. 3. No evidence of intraabdominal bleeding or rupture using vaginal sonography. 4.Diameter of the ectopic mass not greater than 3.5 cm.
Clinical management guidelines for obstetrician-gynecologists. Washington, D.C.: American College of Obstetricians and Gynecologists, 1998. Criteria for medical management (Metrotrexate) Absolute requirementsHemodynamic stabilityUltrasound findings consistent with an ectopic pregnancyWillingness on the part of the patient to adhere to close follow-upNo contraindications to methotrexate therapy Relative requirementsUnruptured ectopic mass less than 3.5 cm in greatest dimensionNo fetal cardiac motion detectedBeta-subunit human chorionic gonadotropin level that does not exceed 5,000 mIU per L (5,000 IU per L)
Surgical management • Salpingostomy / Salpingotomy • The small pregnancy less 2 cm. in length and located in the • distal third of the fallopian tube. • Salpingostomy is currently gold standard of surgical methods • for unruptured ectopic pregnancy • Salpingectomy • uncontrolled bleeding, extensive tubal damage or recurrent • ectopic pregnancy in the same tube. • Complete family or wants a sterilization .
Laparotomy VS Laparoscopy • Hemodynamic stabilty • size and location of mass • surgeon’s experience • Laparoscopy • less recovery time than laparotomy • less operative time • less anesthetic and surgical risk • gold standard for diagnosis ectopic pregnancy
Operative note ( 5 ต.ค.44 ) • Diagnosis - Left tubal pregnancy • Operation - Left salpingectomy • Anesthesia - Spinal block • Position - supine • Incision - Pfannenstiel skin incision • Finding • Hemoperitonium 200 ml. • Left tubal mass at ischemic part size 3*4 cm., • unruptured
Post op. Order for left salpingectomy • นอนราบ 12 hrs. • NPO • record V/S and I/O • 5% D/N/2 1000 ml.. IV 120 cc./hr. * 3 • retained foley’s catheter • Medication:Ampicillin 1 g. IV. q 6 hr. • Morphine 10 mg. IM. q 4 hr.*4 dose • then prn for pain q 4 hr • Plasil 1 amp. IM. prn for N/V q 6 hr.
Pathology report Left fallopian tube,6.5 cm. long and 1.5-2.5 cm. in diameter. The dilated portion is located at proximal part of Fallopian tube, containing dark red material. Diagnosis : Left tubal pregnancy
Ectopic pregnancy Ruptured ectopic pregnancy is the leading cause of maternal mortality in the first trimester and accounts for 10 to 15 percent of all maternal deaths.
Risk Factors for Ectopic Pregnancy Strong evidence for associationPelvic inflammatory diseasePrevious ectopic pregnancyEndometriosisPrevious tubal surgeryPrevious pelvic surgeryInfertility and infertility treatments
Obstetric History - Para 1-0-1-1 Last 8 yrs. Normal labor 2,700 g.
Hct 29.8 % blood gr. B Rh + VDRL non reactive HBsAg -ve Anti HIV +ve - TWG 2.4 kgs.
For treatment : - NPO - 5 % D/N/2 1000cc. + oxytocin 20 u IV drip 100 cc./hr - Methylergonovine 0.2 mg. Rh isoimmunization ) - Observe vaginal bleeding - Record vital sign -
ต่อมา ผู้ป่วยยังมี Active bleeding อยู่ (ก่อน D&C ) loss of consciousness + BP drop 70/35 mmHg PR 120 ครั้ง/min Hct 15 %
Dx : Incomplete abortion with hypovolemic shock with asymptomatic HIV infection wtth anemia
Plan : - M/G และให้ PRC 4 u - NSS 1,000 ml. IV load
At ward(หลังจากทำ D&C ) ผู้ป่วยมี clinical ดีขึ้น หยุด bleed ได้ให้ NSS ต่อจาก ER 1,000 cc. record vaginal bleeding & vital sign , retained foley’s catheter record I&O BP 110/70 mmHg residual urine 300 cc. clear. fluid input 3,000 cc. output 2,000 cc. Hct 25 %
