  5. Patienten (namn) har deltagit i projekt Hälsolyftet/LSH

  6. Välj Enhetens journaltabeller Om det inte finns någon sedan tidigare välj Ny journaltabell

  7. När hela tabellen är ifylld signeras tabellen

  8.
