INTEGRATING CARE THROUGH CLINICAL CASE MANAGEMENT Nicky Hughes Lead for Chronic Conditions Management Brief overview of role of pharmacists in primary care Hope it will give an appreciation of the breath of work that is covered by the medicines management team but have tried to concentrate on key things that we have done that have made big impact to the UHB in terms of quality, patient safety and cost-effectiveness and affected change. It goes with out saying that evidence-based prescribing underpins all the work that is done.
