  14. Students to Be Tested All Florida public school students enrolled in tested grade levels participate, per Section 1008.22(3)(c)8, Florida Statutes (F.S.) Preidentified Student Labels Only the school assessment coordinator and other certified school personnel may open the shrink-wrapped packages and affix labels Secure test materials must not be opened prior to specified date Students are not permitted to assist in preparing secure materials before testing or in organizing and returning materials after testing All secure materials must be returned to locked storage and remain there until testing

Preidentified Student Labels These five elements must be correct: Student Name Student ID Number District Number School Number Grade Level Destroy labels in a secure manner if any of the five elements are incorrect If any other preidentified information is incorrect, report the incorrect information to your school’s data processor

  31. Pre-ID Labels The following six elements MUST be correct for the student label to be used: Student’s Last Name Student’s ID Number District Name School Number Grade Level ELL Status
  32. English Language Learner (ELL) Status The English Language Learner (ELL) category on student grid sheets no longer records the length of time ELLs have been receiving ESOL services but is now updated to record the length of time ELLs have been enrolled in a U.S. school.
  33. Test Invalidation Policies and Procedures
  34. Invalidations A test administrator should discuss any situation involving possible invalidation with the school assessment coordinator, and the situation should be investigated immediately When a student’s test is invalidated for any of the reasons described in this Test Invalidation section, return the invalidated test with the DNS bubble(s) gridded with all other TO BE SCORED materials. This will ensure that the student is counted for participation purposes, but the student will not receive a score.
  35. Guidelines for Appropriate Invalidations A student has an electronic device during testing A student’s test MUST be invalidated A student is cheating during testing Cheating is cause for immediate test invalidation A student becomes ill and is unable to finish, OR a student is not allowed the correct amount of time Invalidation decisions should be made based on whether the student was provided adequate time to respond completely to the test items If a determination can be made that the student was provided enough time to respond completely, the test should be submitted for scoring If it is determined that the student was not provided adequate time to respond completely AND the validity of the test results has been compromised, the test should be invalidated However, if the validity of the test results has not been compromised AND the student has not left the school’s campus, the student may return to the testing room during the same school day to complete the test

Guidelines for Appropriate Invalidations A student is given an accommodation not allowed A student is given an accommodation not indicated on the student’s IEP, Section 504 plan, or ELL plan A student was NOT provided an allowable accommodation indicated on the student’s IEP, Section 504 plan, or ELL plan Should be discussed with the student and his or her parents/guardians to determine if the lack of the accommodation significantly affected the student’s performance and whether the test should be scored

Guidelines for Appropriate Invalidations A student works in the incorrect session If a student working in Session 1 goes on to Session 2 on the same day and it is discovered during testing, the student should be allowed to complete Session 2 with the correct amount of time and the test may be submitted for scoring If this occurs, the student’s parents/guardians should be notified of the misadministration and the student should be asked if he or she is comfortable with his or her performance on the test to determine if the test should be scored If it is discovered after the student has left for the day and Session 2 has not been completed, the test must be invalidated. If a student working in Session 2 of a test goes back and works in Session 1, the test must be invalidated.
  38. Guidelines for Appropriate Invalidations An error occurs in test administration or procedures that could compromise the validity of the test results If the validity of the test results has been compromised (e.g., students had access to an unauthorized visual aid that gave an unfair advantage), the test must be invalidated A disruption occurs during testing If students are disrupted during testing due to a circumstance out of their control (e.g., severe weather), test invalidation may be considered if a student feels his or her performance was significantly affected by the disruption A student is given unauthorized help during testing If a student received unauthorized assistance or had been given an unfair advantage (e.g., a test administrator had told a student to check the answer to a specific item), the student’s test must be invalidated A student is disruptive during testing the school assessment coordinator should determine whether invalidation is an appropriate course of action
  39. Do Not Score (DNS) and UNDO Bubbles The DNS bubble must be gridded when a test is invalidated or when a document is defective and USED If a DNS bubble has been gridded by mistake, erase the DNS bubble and grid the UNDO bubble (except FAA). Then package the document with all other TO BE SCORED materials.

  42. Test Security Policies and Procedures
  43. Test Security Inappropriate actions by school or district personnel before, during, or after test administration may result in consequences such as test invalidations and/or loss of teaching certification Please carefully read: the Test Security Policies and Procedures the Florida Test Security Statute the Florida State Board of Education Test Security Rule All personnel involved in test administration must read and sign the appropriate Test Administration and Security Agreement FCAT test administrators will also be required to sign a Test Administrator Prohibited Activities Agreement indicating that they understand prohibited activities during test administration and possible consequences of inappropriate behavior Test administrators will record their certification numbers on the Test Administrator Prohibited Activities Agreement

Examples of Prohibited Activities Reading the passages, test items or writing prompts before, during, or after testing Revealing the passages, test items or writing prompts Copying the passages, test items or writing prompts Explaining or reading passages, test items or writing prompts for students Changing or otherwise interfering with student responses to test items or performance tasks Copying or reading student responses Causing achievement of schools to be inaccurately measured or reported

Testing Rules Acknowledgment All tests include a Testing Rules Acknowledgment that reads: During the test, you must not talk to other students or make any disturbance look at another student’s Writing folder, planning sheet and or test items allow another student to look at your Writing folder, planning sheet and or test items ask for help responding to the Writing folder, planning sheet and or test items give help to another student in responding to the Writing prompt or test items have notes or scratch paper, other than your planning sheet have any electronic or recording devices in your possession at any time, including breaks, even if you do not use them fail to follow any other instructions given “After the test, you may not discuss the writing prompt/test items with anyone. This includes any type of electronic communication, such as texting, emailing, or posting online, for example, on websites like Facebook, Twitter, or Instagram.If you are found sharing information about the Writing prompt/test items, even without the intent to cheat, your test will be invalidated.” All students and parents/guardians should be made aware of the Testing Rules Acknowledgement and this clarification to the testing rules prior to testing
  47. Testing Rules Acknowledgment Location of Acknowledgment Back of planning sheet – Writing Inside the front cover of the test document – Paper/Pencil After the Welcome Screen – CBT Prior to testing, students must acknowledge the testing rules that the test administrator read to them by clicking or signing below the Testing Rules Acknowledgment. If a student refuses to sign: The test administrator immediately notifies the school assessment coordinator The school assessment coordinator removes the student from the testing room The student is tested on a make-up day If the student still refuses, the student is tested and documentation of their refusal is kept at the school
  48. Electronic Devices-Students School personnel must ensure that students do not have access to their cell phones or other electronic devices at any time during testing, including breaks (e.g., restroom breaks, lunch), if they will return to complete a test session If a student is found to be in possession of ANY electronic device(s) during testing OR during a break, his or her test must be invalidated Students and parents/guardians must be made aware of this policy prior to testing Electronic devices sign must be posted prior to testing where it will be visible to all students Included in the back of every manual
  49. Electronic Devices-Test Administrators/Proctors Test Administrator Prohibited Activities Agreement – “I understand that during testing I may not: Use my cell phone, check email, grade papers, or engage in other activities that will result in my attention not being on students at all times.” Every script states – “At this time, ensure that your own electronic devices are turned off.”

Test Security The security of all test materials must be maintained before, during, and after test administration Under no circumstances are students permitted to assist in preparing secure materials before testing or in organizing and returning materials after testing After ANY administration, initial or make-up, all secure materials must be returned immediately to the test coordinator and placed in locked storage The Test Materials Chain of Custody Form must be maintained at all times (FCAT and EOC Paper/Pencil Only)

Test Security No more than three people should have access to the locked storage room Secure materials MUST never be left unsecured Secure materials MUST not remain in classrooms or be taken off the school’s campus overnight Secure materials MUST never be destroyed (e.g., shredded, thrown in the trash), except for soiled documents Directions for disposing of soiled documents

Test Security Test administrators MUST NOT administer any assessment to their family members Students related to their assigned test administrator should be reassigned to an alternate test administrator Under NO circumstances may a student’s parent/guardian be present in that student’s testing room
  53. Admission of Students to Testing Precautions must be taken at testing sites when students are unknown to the test administrator or other school staff Photo identification, such as a driver’s license or school ID, must be checked before admitting unfamiliar students to a testing room Ensure that students take each subject test only ONCE during each administration In the event that a student takes a subject test more than once and it cannot be determined which test was taken first, both tests will be invalidated Contact prior school if a student registers during the test administration from another district school Contact Assessment to have test materials transferred to new school
  54. Test Administrators/Proctors All test administrators must be certified educators Non-certified school personnel may serve as proctors Non-certified school personnel must NOT be allowed to serve as test administrators Contracted ESE Staff are NOT permitted by the district to proctor or administer statewide assessments. Proctors may NEVER be left in a testing room alone If the room administrator needs to leave the room for any reason, a substitute certified test administrator MUST act as a replacement

Test Administrators/Proctors The FLDOE strongly encourages the use of proctors in all test administrations to help reduce testing irregularities and aid in investigations, if necessary When testing a large group of students, proctors MUST be assigned to the room to assist the test administrator

Chain of Custody Form Used to account for secure paper/pencil test materials at all times Schools are required to maintain a Test Materials Chain of Custody Form The purpose of this form is to track test materials at all times, including: the date the materials arrived at your school the location they will be stored, the dates and times they are handled, and the names of the persons performing various activities involving the materials Schools must retain electronic or hard copies of completed forms for their files. Original hardcopies of the Chain of Custody form must be packaged and returned to the district assessment coordinator in the District Assessment Coordinator ONLY box.

Test Group Code Test group codes are used to identify groups of students tested together May be used during investigations after testing to verify whether students were tested in the same room Schools must use one unique four-digit test group code for both sessions of each subject test for students in each testing room If students are absent for a test session, a different unique code must be used for each make-up session Test administrators record the code with their required administration information and on seating charts

Security Log Each school is required to maintain an accurate Security Log Provided in the test administration manual Anyone who enters a testing room for the purpose of monitoring the test is required to sign the log This applies to test administrators, proctors, and anyone who relieves a test administrator (even for a short break) regardless of how much time he/she spends monitoring a testing room

  70. Seating Chart Schools are required to maintain an accurate seating chart for your testing room. The chart should record the following: Room name/number Grade level/subject Student names and locations in the room during testing Indicate the front and back of the room Direction students are facing Date Session starting time Test administrator’s name Names of proctors (if applicable) Test group code (not CELLA)

Seating Chart If the seating configuration changes during testing, a new seating chart must be created Multiple versions of seating charts are available on the Assessment website or use one of your own design Make copies of the original seating charts for your records Originals of all the seating charts must be returned to your district coordinator of assessment in the District Coordinator ONLY box

Missing Materials Test Coordinators MUST verify that all secure materials on the packing list are received Any mispackaged or missing secure materials MUST be reported to Tom Cornman immediately Individuals responsible for handling secure materials are accountable for the materials assigned to them Schools must investigate any report of missing secure materials If, after a thorough investigation, a secure document is not found, the test coordinator must contact Cherie Boone immediately

Missing Materials After the investigation a written report must be submitted to Cherie Boone The report must include the following, as applicable: the nature of the situation, the time and place of the occurrence, the names of the persons involved, a description of the communication between the district assessment coordinator’s office and school personnel, how the incident was resolved, and what steps are being implemented to avoid future losses If there is reason to believe that a secure document or test content has been stolen, photographed, or copied, the test coordinator MUST contact Cherie Boone immediately

Test Irregularities/Security Breaches Test administrators must report any test irregularities and possible security breaches to the test coordinator immediately If a test irregularity or security breach is identified, the test coordinator must contact Cherie Boone immediately Decisions regarding test invalidation should not be made prior to communicating with Cherie Boone

Test Irregularities/Security Breaches A written report must be submitted to Cherie Boone after an irregularity or security breach has been discovered The report must include the following, as applicable: the nature of the situation, the time and place of the occurrence, the names of the persons involved, a description of the communication between the district assessment coordinator’s office and school personnel, how the incident was resolved, and what steps are being implemented to avoid future irregularities or security breaches

Supervising Test Administrations Do not: talk with students about any test items or passages help students with their answers or plan what to write While you may encourage students to continue working, you may not talk with them about the prompt or test items check through any test documents or computer screens discuss test items or answers with students even after all test materials have been returned and testing has ended return books to students after they have been collected For example: If a student finishes the test before the allotted time has elapsed, the test administrator or proctor: may encourage them to go back and check their work may not read or comment on responses may not provide information that would help infer the correct answer (such as suggesting that the student might want to review their work on specific items)
  77. Supervising Test Administrations Do: Read scripts for administering a test VERBATIM Scripts can be modified when administering test with accommodations (example: a change in the timing of the test)

Supervising Test Administrations If a student reports an error with an item: DO NOT read item note the grade level, content area, form code, item number, and basic student concern (e.g., two possible answers, no correct answer, etc.) encourage the student to reread the item most concerns result from students who don’t read an item carefully encourage the student to answer the item to the best of his/her ability and continue with the test once testing has ended, inform Cherie who in turn will notify the FDOE with this information The item will be investigated

  83. Scribes The role of the scribe is to write only what is dictated (said or signed). He or she must record word-for-word (VERBATIM), exactly as the student dictates including those dictating through ASL The scribe must not edit or alter student work in any way The scribe must not elaborate on what is being written/chosen and must be careful not to give clues that assist the student or alter the outcome of a test in any way The student must indicate to the scribe the use of capitalization, punctuation, and the spelling of unusual or uncommon words. Words that appear on the Dolch list do not have to be spelled The scribe must allow the student to review and edit what the scribe has written/chosen

Make-up Testing Students who are not tested on the regular administration date due to a temporary illness, disabling condition, etc., should be tested on a make-up day All security and administration procedures must be followed while conducting make-up tests For all FCAT 2.0 RMS tests, Session 1 MUST be completed before Session 2. Any students absent for Session 1 may not participate in Session 2 until they have completed Session 1.

Important Information If a student begins a session and leaves campus without finishing (due to an appointment, illness, etc.), he or she will NOT be allowed to complete that session No exceptions will be made once the student leaves the school’s campus, so parents/guardians and students must be made aware of this policy If you have concerns about a student, you should not allow the student to begin - arrange to administer the test to that student on a make-up day

Family Letters Updated Family Letters have been created so schools may provide parents/guardians a written notification regarding student participation in the test administration These letter may be modified to meet the needs of your school, but the factual content regarding policies should not be altered Letters for Writing are currently available on the Assessment homepage Letters will be posted as they become available All letters will be translated into Creole, Spanish and Portuguese

Approved Four-Function Calculators Schools may only use the four-function calculator provided by the district For use only on the following assessments: Mathematics tests in grades 7 and 8 Science test in grade 8 EOC Algebra EOC Biology The school assessment coordinator and test administrators must ensure that one working calculator is available for each student in the testing room Schools are responsible for storage and security DO NOT throw broken calculators away Return to the Test Distribution Center for replacement or removal from your inventory Contact the Test Distribution Center if you need additional
