NTSB Recommendations • R-02-18: Adopt the APTA manual that contains updated language on auditing the effectiveness of operating rule compliance programs, and simultaneously modify 49 CFR Part 659 so that the Part always references the current APTA manual • R-02-19: Require that new or rehabilitated vehicles funded by FTA grants be equipped with event recorders meeting IEEE Standard 1482.1 for rail transit vehicle recorders • R-06-3: Require transit agencies, through the system safety program and hazard management process if necessary, to ensure that the time off between daily tours of duty, including regular and overtime assignments, allows train operators to obtain at least 8 hours of uninterrupted sleep

NTSB Recommendations • R-06-4: Assess the adequacy of WMATA’s current organizational structure and ensure that it effectively identifies and addresses safety issues • R-06-5: Develop transit railcar design standards to provide adequate means for safe and rapid emergency responder entry and passenger evacuation • R-06-6: Develop minimum crashworthiness standards to prevent the telescoping of transit railcars in collisions and establish a timetable for removing equipment that cannot be modified to meet the new standards

NTSB Recommendations • R-07-9: Modify your program to ensure that State safety oversight agencies take action to prompt rail transit agencies to correct all safety deficiencies that are identified as a result of oversight inspections and safety reviews, regardless of whether those deficiencies are labeled as “findings,” “observations,” or some other term • R-07-10: Develop and implement an action plan, including provisions for technical and financial resources as necessary, to enhance the effectiveness of State safety oversight programs to identify safety deficiencies and to ensure that those deficiencies are corrected

NTSB Recommendations • R-07-11: Schedule the CTA as a priority for receiving the maintenance oversight workshop and the training course to be developed for track inspectors and supervisors that will address the unique demands of track inspection in the rail transit environment • R-07-12: Inform all rail transit agencies about the circumstances of the July 11, 2006, CTA subway accident and urge them to examine and improve, as necessary, their ability to communicate with passengers and perform emergency evacuations from their tunnel systems, including the ability to (1) identify the exact location of a train, (2) locate a specific call box, and (3) remove smoke from their tunnel systems
