  2. CLASIFICACION • Fracturas de clavicula, incluyendo cualquier tercio • Fracturas del tercio proximal del humero, por arriba del cuello quirurgico • Fracturas del cuerpo de la escapula, comprende cavidad glenoidea

  3. FRACTURAS DE CLAVICULA • Puede afectarse cualquier tercio • Independientemente del trazo • Trauma obstetrico • Contusion directa

  4. INDICACIONES QUIRURGICAS ABSOLUTAS • Lesion bilateral de claviculas • Lesion neurovascular • Fracturas expuestas, sin importar el grado • Pacientes polifracturados • Pacientes con traumatismo craneoencefalico

  5. MANEJO CONSERVADOR • Inmovilizador universal de hombro • Inmovilizador en ocho • Vendaje tipo Velpaou • Vendaje tipo Gill Craish

  6. FRACTURAS TERCIO PROXIMAL DE HUMERO • Aquellas lesiones a nivel del cuallo quirurgico y/o por arriba de este • Por lo regular trazos transversos • Pacientes seniles presentan multifragmentacion

  7. TRATAMIENTO • Quirurgico, solo en pacientes jovenes, independientemente del trazo • Conservador, pacientes de la tercera edad a base de cualquier tipo de inmovilizador, procurando siempre cuidar el pliegue de la axila y piel

  8. FRACTURAS DE ESCAPULA • Fracturas por alta energia • Frecuentemente a nivel del cuerpo y el cuello de la escapula • Descartar del lado derecho contusion pulmonar y del izquierdo contusion miocardica ( ECG Enzimas cardiacas Obs en Urg Tele de torax )

  9. HOMBRO FLOTANTE • Cuando se encuentran fracturados dos huesos de la cintura escapular al mismo tiempo

  10. TRATAMIENTO DEL HOMBRO FLOTANTE • Siempre se volvera quirurgico, independiente de los trazos y edad del paciente • La prioridad es estabilizar el mayor numero de fracturas y mejorar capacidad pulmonar
