When mishap prevention efforts fail, and mishaps occur, a thorough investigation is necessary to prevent recurrence. There are various classes of mishaps, based on severity of injury or costs of damage. Class A mishaps, which involve a fatality, permanent total disability, or over $1,000,000.00 damage must be investigated by a formal mishap investigation board. All other mishaps, no matter how minor, must be investigated by the command. The Safety Officer may assist a mishap investigation board or conduct other shipboard investigations, and must understand mishap investigation techniques, limitations, and the limited use and/or protection of safety information Lesson Overview.
