About CCBRT HISTORY Vision 1994: CCBRT established to provide Community Based Rehabilitation (CBR) 1996:CBR services extended to Moshi 2001: Disability Hospital (DH) opened in Dar es Salaam on land donated by the Government of Tanzania (GoT) 2006:DH surgical capacity expanded 2007: CCBRT and GoT formalized a public-private partnership to establish and operate Maternity and Newborn Hospital (MH) 2011: CCBRT broke ground on MH A Tanzania where people have access to quality disability services as well as safe maternal and newborn healthcare

  5. Solution: TransportMyPatient Overview Impact • Consistent increase in the number of patients accessing treatment • On track for 1,000 in 2015; 339 in 2011 • Correct referrals to CCBRT • Patients seeking treatment from remote locations

Solution: Lean Management • Overview • Equip staff with skills to use scientific approach to problem solving • Standardize existing treatment and process protocols, procedures, and trainings • Coordinate monitoring and evaluation frameworks • Pilot implementation of the Integrated Management System • Impact • Continuous quality improvement process implemented in eye department; patient flow revised
