  2. Overview of SOAR in NC NC Coalition to End Homelessness

History of SOAR in NC • NC became a SOAR State in 2007 • Initial strategy to train large numbers of case managers and direct service providers • 2009 – began to focus on developing and training dedicated SOAR caseworker positions within programs • PATH Teams/Homeless service providers • Community mental/medical health clinics • DSS North Carolina Coalition to End Homelessness

  6. NCCEH and NC SOAR NCCEH is contracted through the state’s ICCHP to provide training and technical assistance for SOAR caseworkers and communities. Caseworkers: • 2 day SOAR Training • SOAR Caseworker Certification • Monthly SOAR Dialogue Phone Calls • Ongoing TA Communities: • SOAR Essentials training for overview of SSI/SSDI and SOAR • SOAR Community Certification • TA provided to communities to start Disability Workgroups in order to develop community supports for SOAR case workers • Facilitate trainings for doctors and other treatment providers on documenting disabilities NC Coalition to End Homelessness

SOAR Caseworker Certification • NC Coalition to End Homelessness created process in September 2009 • Slow to start, but gaining momentum as NC creates more dedicated SOAR Caseworker positions • Currently have 6 caseworkers certified and 3 in process • Certification process is now included in job descriptions for some dedicated SOAR Caseworkers NC Coalition to End Homelessness

Goals of Certification Process • Provides a way to recognize SOAR Caseworkers who use their SOAR training in the field • Provides an opportunity for NCCEH to review Medical Summary Reports for quality review purposes • Provides an opportunity for one-on-one technical assistance with SOAR Caseworkers • Reinforces NC SOAR process by using Quality Review checklist for applications and reporting outcomes • Provides NCCEH with feedback on TA and SOAR process NC Coalition to End Homelessness

Other Benefits • Improves Medical Summary Reports • Encourages caseworkers to use SOAR after training and recognizes their hard work • Provides state team lead with examples of good Medical Summary Reports for trainings • Certification requirements for caseworkers are easy to include in job expectations for dedicated SOAR Caseworker positions NC Coalition to End Homelessness

SOAR Caseworker Certification To become an NC SOAR Certified Caseworker and receive a Certificate, an individual must complete two stages of certification: STAGE 1 • Attend the two-day SOAR training for caseworkers • Complete at least one SOAR application using Quality Review Checklist • Submit copy of Medical Summary Report and Quality Review Checklist to NCCEH • Submit SOAR outcomes form to NCCEH • Complete SOAR Caseworker Certification Exam Completion of Stage 1: NC SOAR Certified Caseworker—Provisional Status NC Coalition to End Homelessness

SOAR Caseworker Certification Medical Summary Report Rubric: • Identifying information of applicant (REQUIRED) • Full Name • Date of Birth • Full Social Security Number • Introductory paragraph that provides a clear physical description (REQUIRED) • Personal History • Childhood description including any abuse history • School history • Work history • Relationship history • Homelessness history • Medical History (REQUIRED IF USING PHYSICAL IMPAIRMENT LISTING) • List of symptoms experienced and described by individual (using SSA listing for guidance) • Medical Treatment History (citing records obtained by case worker) NC Coalition to End Homelessness

SOAR Caseworker Certification Medical Summary Report Rubric: • Mental Health History (REQUIRED IF USING MENTAL HEALTH LISTING) • List of symptoms experienced and described by individual (using SSA listing for guidance) • Mental Health Treatment History (citing records obtained by case worker) • Substance Abuse History (REQUIRED IF SUBSTANCE ABUSE IN MEDICAL RECORDS) • History of use including when started, point of heaviest use, periods of abstinence • Explanation of how the Substance Abuse does or does not impact the symptoms experienced including if the individual uses substances to manage current symptoms • Functioning Information (REQUIRED) • Clearly defined paragraphs that address specific examples of functioning impairment in: • Activities of Daily Living • Social Functioning • Concentration, Persistence, and Pace • Repeated Episodes of Decompensation • Contact information for SOAR Case Worker and treating physician (REQUIRED) NC Coalition to End Homelessness

SOAR Caseworker Certification SOAR Caseworker Certification Exam: • Available on NCCEH’s website • 14 questions (T/F, open-ended, multiple choice) • Pre- and Post-Test questions from the SOAR Trainings • Additional questions address: • Applicant self-determination • Clear use of language with applicants • Cultural differences and how caseworkers address these differences with applicants NC Coalition to End Homelessness

SOAR Caseworker Certification To become an NC SOAR Certified Caseworker and receive a Certificate, an individual must complete two stages of certification: STAGE 2 • Complete 5 additional applications • Submit copies of all medical summary reports, quality review checklists and outcomes to NCCEH for review • Complete self-assessment and evaluation of SOAR values, skills and experience NC Coalition to End Homelessness

SOAR Case Worker Certification SOAR Case Worker Self-Assessment and Evaluation: • 6 open-ended questions • Available on NCCEH’s website • Questions address: • Reflections on completing SOAR applications • Overall experience with SSA and DDS • Feedback on NCCEH’s technical assistance NC Coalition to End Homelessness

SOAR Community Certification • NC Coalition to End Homelessness created process in March 2009 • Three levels: Bronze, Silver, Gold • Currently have: • 1 Gold Community • 1 Silver Community • 3 Communities working towards Bronze • Certification process can be included in community Ten Year Plans to End Homelessness NC Coalition to End Homelessness

Goals of Certification Process • Strengthens buy-in for SOAR in communities • Provides clear goals for communities to work toward in developing successful SOAR programs • SOAR caseworkers receive assistance from community partners in advocating for systems change • Develops SOAR community work groups to provide SOAR projects with: • Quality Review • Governance • Mentoring NC Coalition to End Homelessness

SOAR Community Certification Sample of Certification Requirements: • Bronze: Designate SOAR Implementation Lead/Team. • Silver: Community has a dedicated, NC SOAR Certified staff person who assists with applications, serves as SOAR Community Coordinator and acts as the SSI/SSDI expert for the community. • Gold: Community continues to build and strengthen relationships with and provide education for community partners, including provider agencies, clinics, hospitals, funders, etc. NC Coalition to End Homelessness

SOAR Dialogue Phone Calls • NCCEH holds conference calls once a month for SOAR Caseworkers from across the state • Discussions are caseworker-driven with caseworkers suggesting speakers/topics • Phone calls include updates from communities and time to address individual concerns • Discuss a variety of topics: • Developing relationships with doctors • Presumptive Disability decisions • How to recognize TBI NC Coalition to End Homelessness

Goals for Dialogue Phone Calls • Provides a way to mentor and network with other SOAR Caseworkers • Provides a way for NCCEH to receive feedback on SOAR process and technical assistance needs • Allows NCCEH to disseminate information quickly to SOAR Caseworkers in the state NC Coalition to End Homelessness
