  1. Carer’s Assessment Case Study A presentation by Keith Lugton (Pre-hospital discharge Carer Support Worker) and Roslyn Scott (post-hospital discharge Carer Support Worker)

  2. The carer… John, 49 years old, unemployed. Suffers depression and stress anxiety. Receives incapacity benefit and a carer premium (underlying entitlement to carer’s allowance). Lives in Glasgow.

  3. The caree… John’s father, 81 years old. In hospital with COPD followed by diagnosis of terminal lung cancer. John’s mother, 81 years old. History of bowel cancer and hip trauma (frequent dislocations after THR surgery). Both live in Edinburgh.

  4. Carer meeting… Following the meeting a plan of support was agreed. CSW to refer carer for a Carer’s Assessment as part of support plan.

  5. Who is responsible for assessment…? The local authority where the carer lives (ordinary residence)? Local SWC in Glasgow.  Local carers support project in Glasgow.  Post discharge CSW in Edinburgh.

  6. Who is responsible for assessment…? Community Care & Health (Scotland) Act 2002: • Joint working between two local authorities. • The local authority where the caree lives should take the lead in assessing the carer.

  7. Post-discharge carer support… • VOCAL Counselling service referral • Glasgow North West Carers Pack • Telephone / E-mail support • Linking in to Swift system • Home visits to meet the family and offer 1-1 support • Completed carer’s self-assessment form
