Overview of Busy Agenda • Identify the Sustain Leader to monitor the data. • Identify the Data Hawk who will collect the data and send to Sustain Leader. • Determine the lower threshold for action, the time frame and data point for the Change Team to reconvene • Convert the Change Project Form into Clinical Protocol for Sheridan, CORE, TASC, and Adult Parole delineating roles and responsibilities; • Make Recommendations for Spread of process to other sites • Disseminate Change Team Results within involved agencies; • Create storyboard (with graphs and photos), for telling the story; • Establish anniversary date for group to discuss Sustain Success; • Discuss other major shared issues for potential Change Teams.
