Unlocking the Potential of Community Pharmacy

The Nottinghamshire & Derbyshire GP/Pharmacy Transformation Programme aims to capitalize on the untapped potential of community pharmacy to improve primary care access, release GP time, improve patient experience, and achieve better health outcomes. This pilot project, funded by NHS England, has shown positive outcomes, including significant clinical interventions, GP time saved, and potential reduction in secondary care referrals. The next steps include exploring a business case to expand the model for Community Pharmacist Independent Prescribers in General Practice, with the aim of sustainability and workforce development.

Background Business case to explore ‘untapped potential’ of Community pharmacist independent prescribers in supporting GPs (July 2014) NHS England local office funded 18 month project Hosted and managed by Newark & Sherwood CCG Initially 4 sites across both Derbyshire & Nottinghamshire Extended to 6 sites

Project Overview and Progress • First site went live July 2015 • Implementation over 9 months, final site started April 2016 • Pharmacists undertaking clinical reviews: • Medication reviews including care homes (56%) • Long term disease management reviews (asthma, CHD, CKD and hypertension) (29%) • Urgent care (7%) • Other (8%) • ‘Other’ includes medication queries, discharge information, travel queries, anticoagulant review

Emerging Outcomes • At end August 2016: • 6400 consultations – face-to-face and telephone • Estimated 1,250 hours of GP time saved • Favourable cost per consultation - £21.00 to £38.00 • Evidence of significant clinical interventions • Medicines changed in 59% of cases • Safety and quality improvements • side-effects18%, blood tests 15%, self-care advice 23% • 8.5% potential reduction in secondary care referrals • Excellent patient feedback

  8. Next Steps and Sustainability • Project is delivering positive outcomes but still to reach its full potential • Due to end November 2016, but each site will remain live for at least one year • Project now runs alongside National Clinical Pharmacists in GP practice pilot • Business case being explored to extend pilot and expand model for Community Pharmacist Independent Prescribers in General Practice • Sustainability will require coherent planning and workforce development
