  2. We introduced ourselves and went through the meeting map to clarify purpose, roles rules and agenda.

  3. “You spend your whole life making decisions about things – your work, your relationships, your children – you don’t want to suddenly give up that responsibility because you’re older.”

  4. We looked at how we can use the person centred thinking skills to explore how we could make self directed support a reality for older people

  5. The person centred thinking triangle Supporting Dreams Supporting Relationships, Community Connecting • Learning Log • The 4 Questions • Working/Not Working Learning, Using and Recording Communication Matching Staff and Those Using Services Recognizing and Sorting Important To and Important For & Finding the Balance Between Them Defining Staff Roles and Responsibilities

  6. We looked at how a one page profile can grow and evolve over time, changing things that can make a difference to people straight away. We heard about Arthur and Nora’s story

  7. We shared different ways of capturing histories….’with history comes regard’ – seeing older people in the context of their lives & experiences

  8. We looked at the Context in terms of the National picture and what is happening locally that may affect this work

  9. We then looked at what success would look like for: older people, families, services, staff, others………

  10. …….what will be different and for whom? Then began the hard work developing our plan to move towards success!!

  11. Where/on what do you want to focus attention & developments on person centred thinking & planning with older people – and why?What would success look like….We transferred the success indicators from card call to our poster

  12. Which person centred tools might help to achieve success? Who could help this to happen in Tameside? What could help this to happen in Tameside? Who or what might get in the way?

  13. How do you want to use your support days over the next 12 months? Agreed Next Steps.. How are we going to share what we are learning? Julie overall co-ordination booking venues & logistics for training, learning and reflection, and process mapping days

Closing Comments • This touches all of the bases we have been thinking about for a long time • How exciting- we have been waiting for so long to make major changes and this will make them happen • Quite scary to see this happen in such an organised manner • This takes me to what social work is about – why I came in to the work. • Great step forward- it feels like the answer • We all get a say- I am really taken aback by the day • We have a shared vision • I am very excited- clearly we are all on the same side- today has proved this • This is a real opportunity to enhance the quality of life of the people we support • Connection with Oakwood House is fantastic! • Overwhelmed but excited!
