Our Service Model • Medicare’s Health & Behavior CPT codes are used to address psychological complications from medical conditions • Follows a brief, problem-focused treatment model, and shifting away from traditional, long term psychotherapy • Promotes integrated, collaborative care with IDT • Helps patients understand how physical condition is impacted by emotions, outlook, and behavior • Helps families understand their role in bringing about patient change
