As a Contest Hole Sponsor you are required to have staff at the hole to monitor contest. Shared Hole Sponsor Investment $50 • Hole sponsor will have signage on a hole

COURSE SPONSORSHIPS Sponsorship Form Hole Options As a savvy hole sponsor you want to make the most of your investment, so here are some items to make your sponsorship memorable to all participants. Beer Keg Investment $500 If you would like to serve golf participants beer, at the hole you are sponsoring, you can have a beer keg at your hole. This option includes one keg of beer, a server, and cups. If you would like to have the beer served in your own promotional cup, the course server can serve in your provided cup, as long as the cup is plastic. Table Investment $50 As a Hole Sponsor you can get the most from your hole sponsorship by having a display table at your hole. If you would like a table provided by the golf course, one can be provide at a minimal investment. Practice Green Contest Sponsor* Investment $200 • Practice Green Contest Sponsor will have exclusive signage at the practice putting green • Display table at the hole • Name on www.bgcna.org website as a Practice Green Contest Sponsor • Name on award plaque for contest *Note: As a sponsor of a hole you may have a display table, canopy, and additional signage at the hole.

COURSE SPONSORSHIPS Sponsorship Form Refreshment Cart Sponsor Investment $1000 • Signage on Refreshment Cart • Logo on www.bgcna.org website as Refreshment Cart Sponsor Quantity Available: Two (2) Photography Sponsor Investment * As the Photography Sponsor you are expected to take photos of all of the day’s events and pictures of all of the foursomes playing golf in the event, along with providing pictures for each person in a foursome in a photo holder and digital copies of all photos to the Boys and Girls Club of Naperville. This Sponsorship includes: • Marking the back of photos and holder • Logo on www.bgcna.org website as Photography Sponsor • Display table at dinner Quantity Available: One (1) *Note: Donation of all Photography to BGCNA Specifications.

GENERAL SPONSORSHIP Sponsorship FormINFORMATION Application and Payment Due* • Sponsorship form and payment due July 26, 2013 • Early Bird rates end July 26, 2013 • Standard rates begin July 27, 2013 • Standard payment due August 1, 2013 Additional Tickets If you would like to purchase additional rounds of golf and/or additional tickets for dinner, as a sponsor you will receive a special sponsor early bird rate if purchased prior to July 15, 2012. Dinner ticket includes four free raffle tickets ($20 Value). • Early Bird Round of Golf and Dinner $125.00 • Round of Golf and Dinner $150.00 • Ticket for Dinner Only $ 50.00 Logo Requirements If your sponsorship includes the use of your corporate logo, please include a letter of authorization to: asmith@bgcna.org. (Logo will be omitted without authorization letter.)
